Periprothetische Acetabulumfrakturen

Zusammenfassung: Periprothetische Acetabulumfrakturen sind seltene, jedoch epidemiologisch bedingt zunehmende Verletzungen bzw. Komplikationen in der Endoprothetik des Hüftgelenkes, die ein oft multimorbides und geriatrisches Patientenkollektiv mit schlechter Knochenqualität betreffen. Man unterscheidet intraoperative von postoperativen Frakturen, die entweder traumatisch akut bedingt sind oder z.B. assoziiert mit vorbestehenden Osteolysen im Rahmen von Niedrigenergietraumata im Sinne von Insuffizienzfrakturen auftreten können. Die intraoperative Inzidenz beträgt bis zu 0,4% und eine zementierte Press-fit-Versorgung ist mit einem höheren Risiko assoziiert. Die Diagnostik sollte standardmäßig Röntgen und CT umfassen und ggf. durch einen Infektausschluss bei schleichenden Frakturen ergänzt werden. Das zu wählende Behandlungskonzept sollte Faktoren wie Frakturdislokation, Implantatstabilität, Knochenqualität inkl. etwaiger Defekte, funktionellen Patientenanspruch und auch Erfahrung des chirurgischen Teams mit Erfahrung in der Acetabulumchirurgie als auch Revisionsendoprothetik inklusive Verfügbarkeit eines entsprechenden Implantatportfolios beachten. Die konservative Therapie kommt selten zum Einsatz. Operativ sind die Osteosynthese des hinteren Pfeilers und eine stabile Pfannenversorgung elementar. Hierfür stehen diverse Zugänge als auch Implantatoptionen zur Verfügung. Die Therapie periprothetischer Acetabulumfrakturen ist anspruchsvoll und sollte an einem spezialisierten Zentrum erfolgen. Dieser Artikel soll einen Überblick über die aktuellen Standards zur Diagnostik inklusive Klassifikationsoptionen und die verschiedenen Therapieoptionen wie auch Algorithmen zur Versorgung periprothetischer Acetabulumfrakturen geben.

Summary: Periprosthetic acetabular fractures are rare, but due to epidemiological reasons increasing injuries or complications in the endoprosthetic surgery of the hip joint, which affect an often multimorbid and geriatric patient population with poor bone quality. One has to differentiate between intraoperative and postoperative fractures, which either occur due to traumatic reasons or are, for example, associated with pre-existing osteolysis occurring in the context of low-energy trauma in the sense of insufficiency fractures. The intraoperative incidence is reported to be up to 0.4% and cemented press-fit surgery is associated with a higher risk. Diagnostics should include X-ray and CT as standard and, if necessary, be supplemented by infection diagnostics in the case of osteolytic fractures. The treatment concept to be selected should take into account factors such as fracture dislocation, implant stability, bone quality including any defects, functional patient demands and also the experience of the surgical team with experience in acetabular surgery and revision arthroplasty as well including the availability of a corresponding implant and hardware portfolio. Conservative therapy is rarely applied. Surgically, osteosynthesis of the posterior wall and a stable cup restoration are elementary. Various approaches and implant options are available for this purpose. The therapy of periprosthetic acetabular fractures is demanding and should be performed at a specialized center. This article is intended to provide an overview of the current diagnostic standards including classification options and the different treatment options and algorithms for the treatment of periprosthetic acetabular fractures.

Operative Therapie von Patella-Frakturen

Zusammenfassung: Frakturen der Patella sind eine seltene Entität, deren Versorgung relativ komplex sein kann. Zur Bestimmung der exakten Frakturmorphologie und zur Operationsplanung ist eine Computertomographie empfehlenswert. Frakturen mit einer Dislokation über 2mm sowie Mehrfragmentfrakturen, offene Frakturen oder osteochondrale Frakturen sollten operativ behandelt werden. Zur operativen Versorgung stehen mehrere Osteosyntheseverfahren zur Auswahl. Die klassische Zuggurtung ist nach wie vor am meisten genutzt, wenn auch moderne winkelstabile Plattensysteme bessere biomechanische Ergebnisse zeigen konnten. Für Querfrakturen eignet sich bei guter Knochenqualität die kanülierte Schraubenosteosynthese, die mit einer Zuggurtung kombinierbar ist. Trotz einer zufriedenstellenden Reposition sind die Langzeitergebnisse, bezogen auf die Patientenzufriedenheit, als mittelmäßig einzustufen.

Summary: Only 1% of all fractures happen to the patella. The treatment of patella fractures can be very complex and difficult. For the exact fracture mapping and the ideal preoperative planning a CT-scan should be obtained. Fractures with a dislocation of more than 2mm, multifragment fractures, open fractures or osteochondral fractures should be treated operatively. There are different types of osteosynthesis to choose from. The traditional tension wire osteosynthesis is the most popular, even though modern locking plates turned out to be biomechanically superior. Transverse fractures of the patella can be treated with cannulated screws in combination with tension band wiring. Despite good reduction and reposition of the fracture many patients show persistent complaints in the long-term subjective postoperative outcome.

Minimal invasive Rekonstruktion des medialen patellofemoralen Ligaments (MPFL) mit der Quadrizepssehne

Zusammenfassung: Zur Rekonstruktion des medialen patellofemoralen Ligaments (MPFL) wurden zahlreiche Operationstechniken publiziert, wobei vor allem die Sehnen von M. Semitendinosus oder M.Gracilis als Transplantat verwendet wurden. Trotz überwiegend sehr guter klinischer Ergebnisse dieser Operationsmethoden, haben sich auch Komplikationen wie bspw. Patellafrakturen (durch Bohrkanäle oder Implantate) gezeigt. Bei Rekonstruktionstechniken, die ein Quadrizepssehnentransplantat (QS) verwenden, kann dagegen auf Knochenkanäle an der Patella verzichtet werden. Mehrere Studien über MPFL-Rekonstruktion mit der QS konnten sehr gute klinische und funktionelle Ergebnisse bei niedrigerem Komplikationspotential zeigen. Parallel dazu ergaben experimentelle Studien eine größere Ähnlichkeit biomechanischer Parameter des QS-Transplantats zum unverletzten MPFL als Hamstrings-Transplantate (HS). Wir beschreiben eine minimal invasive MPFL-Rekonstruktionstechnik mit einem Quadrizepssehnenstreifen. Diese Technik stellt eine gute bohrkanalsparende Alternative zu den weit verbreiteten HS-Techniken dar, und ist sowohl für primäre und Revisionseingriffe bei Kindern und Erwachsenen geeignet. Außerdem stellt diese Technik eine bohrkanalsparende Alternative für Kinder mit offenen Wachstumsfugen dar.

Summary: Hamstring tendon (HT) MPFL reconstruction techniques have been shown to successfully restore patella stability, but complications including patella fractures have been reported. Quadriceps tendon (QT) reconstruction techniques with an intact graft pedicle on the patella side have the advantage that patella bone tunnel drillings and implant fixations are no longer needed, reducing risk of patella fractures. Several QT MPFL reconstruction techniques, including minimally invasive surgical (MIS) approaches, have been published with promising clinical results and fewer complications than with HT techniques. Parallel laboratory studies have shown macroscopic anatomy and biomechanical properties of QT are more similar to native MPFL than HT, suggesting QT may more accurately restore native joint kinematics. We describe a minimal invasive MPFL reconstruction technique using a strip of QT. This technique demonstrates promising clinical results and offers a valuable alternative to HT grafts for primary and revision MPFL reconstruction in both children and adults.

Komplikationen und Revisionsstrategie nach Trochleaplastik

Zusammenfassung:

In der Entstehung der patellofemoralen Instabilität stellt die Trochleadysplasie einen der wichtigsten anatomischen Risikofaktoren für rezidivierende Luxationen dar. Durch die Insuffizienz der statischen Stabilisation kann es bei ausgeprägter Dysplasie zu einem vollständigen Verlust des Patellatrackings kommen. Hier stellt die Trochleaplastik eine der effektivsten Behandlungsmöglichkeiten zur Wiederherstellung des Patellatrackings dar. Neben dem hohen technischen Niveau birgt der Eingriff jedoch ein beachtliches Risikoprofil, bedingt durch die Komplexität der Pathologie und die Invasivität des Eingriffs.

Neben Herausforderungen bei der Indikationsstellung, gehören im postoperativen Verlauf rezidivierende Instabilitäten, ein gesteigertes Schmerzniveau, persistierende Bewegungseinschränkungen und die Entwicklung patellofemoraler Arthrose zu den häufigsten Gründen für eine Revisionsoperation. Unter Berücksichtigung möglicher Fallstricke bei der Indikationsstellung und nach gründlicher Analyse der individuellen Pathologien ist die Trochleaplastik ein effektives Werkzeug zur Therapie der patellofemoralen Instabilität. Das beschriebene Risikoprofil macht eine vergleichsweise engmaschige Nachbehandlung empfehlenswert.

Summary: Femoral trochlear dysplasia is considered to be one of the most important anatomical risk factors for recurring episodes of patellofemoral instability. This structural and functional insufficiency of static stabilization can consequently result in the total loss patellar tracking. Therefore, in severe types of dysplasia, trochleoplasty can be an effective treatment option. However, the different surgical techniques require a high level of surgical skills. They bear a considerable risk profile, which arises from the complexity of the deformity and from the invasiveness of the procedure itself.

While sometimes, the indication itself can be challenging, various postoperative complications such as recurring patellar dislocations, an increased level of pain, patellofemoral osteoarthritis or a limited range of motion are the most common reasons for revision surgery. In severe cases of trochlear dysplasia, trochleoplasty can be an effective treatment option. This, however, requires the consideration of certain pitfalls, a thorough preoperative analysis of individual pathologies and a closely monitored follow-up.

Radiologische Diagnostik und Klassifikationen femuropatellarer Dysplasien

Zusammenfassung: Die femoropatellare Instabilität gilt als prädisponierender Faktor einer femuropatellaren Arthrose und ist multifaktoriell bedingt. Die entsprechenden Elemente umfassen die Muskulatur, kapsulo-ligamentäre und knöcherne Strukturen. Dieser Artikel behandelt insbesondere die knöchernen Faktoren, genauer genommen die Dysplasie der Patella und der Trochlea. Radiologische Klassifikationen dienen hierbei primär der Bewertung der einzelnen Risikofaktoren der femuropatellaren Instabilität. Darüber hinaus trägt die radiologische Bildgebung zur Charakterisierung von zugrunde liegenden anatomischen Anomalien, Patellafehlstellungen und möglichen Weichteilverletzungen bei. Die Ergebnisse dieser Untersuchungen können orthopädischen Chirurgen bei der Therapieentscheidung und dem post-operativen Monitoring unterstützen.

Summary: Patellofemoral instability is multifactorial and considered to be a predisposing factor for patellofemoral arthrosis. The corresponding elements include the musculature, capsulo-ligamentous and bony structures. This article deals in particular with the bony factors, more precisely with dysplasia of the patella and trochlea. Radiological classifications are primarily used to assess the individual risk factors of femuropatellar instability. Furthermore, radiologic imaging contributes to the characterization of underlying anatomic abnormalities, patellar malalignment, and possible soft-tissue injuries. The results of these examinations can aid orthopedic surgeons in decision making when to operate and to monitor patients postoperatively.

Konservative Behandlung patellofemoraler Pathologien

Zusammenfassung:

Patellofemorale Schmerzen umfassen ein breites Spektrum an Pathologien, darunter das patellofemorale Schmerzsyndrom (engl.: patellofemoral pain syndrome, PFPS), welche häufig auf mechanische Überlastungen und Sagittalachsabweichungen der unteren Extremität mit lokalen, proximalen und distalen ätiologischen Faktoren zurückzuführen sind. Diese systematische Übersichtsarbeit identifiziert 6 Behandlungsmodalitäten im Rahmen des konservativen PFPS-Managements: Physiotherapie, Übungs- und Rehabilitationsprogramme, Taping, Elektrotherapie, Schuheinlagen und Patellofemoral (PF)-Orthesen. Resultierend konnte keine Überlegenheit einer einzelnen Behandlungsmethode gegenüber den anderen Behandlungsansätzen identifiziert werden, da die multifaktorielle Genese des PFPS verschiedene Ansätze erfordert. Der Fokus der identifizierten Behandlungsprotokolle lag auf der Aktivität und einer gezielten Kräftigung der unteren Extremität. Dabei schien der zusätzliche Einsatz von Taping und biomechanischen Interventionen wie Orthesen mit Einfluss auf das Patella-Alignment während der Aktivität zu einer Schmerzreduktion und zu einer positiven Erfahrung mit einem langfristigen Erfolg beizutragen.

Summary: Patellofemoral pain covers a wide range of pathologies, including patellofemoral pain syndrome (PFPS), which is often the result of mechanical overload and sagittal axial deviation of the lower extremity with local, proximal and distal factors of etiology. This systematic review identifies 6 treatment modalities within the framework of conservative PFPS management: physiotherapy, exercise and rehabilitation programs, taping, electrotherapy, shoe inserts and patellofemoral (PF) orthoses. As a result, no single treatment modality could be derived, since the multifactorial etiology of PFPS requires different approaches. The focus of the identified treatment protocols was on activity and strengthening programs of the lower extremity, whereas the additional use of taping and biomechanical interventions such as orthoses during activity can lead to beneficial effects. The influence on patella alignment seemed to contribute to pain reduction and thus to a positive experience with long-term success.

Der patellofemorale Schmerz

Zusammenfassung: Der patellofemorale Schmerz (PFP) ist ein häufig auftretendes Schmerzphänomen, dessen Ursache multifaktoriell ist. Anatomische, funktionelle, biomechanische u.a. Faktoren spielen in der Genese eine Rolle. Ein möglicher Einfluss von sozialen und psychologischen Aspekten ist unklar.

Im Rahmen der Diagnostik haben vor allem die ausführliche Anamnese sowie die klinische Untersuchung mit funktionellen Tests und Übungen entscheidende Bedeutung. Neben dem Ausschluss intraartikulärer Pathologien durch eine entsprechende Bildgebung gilt es zudem, ein anatomisches oder funktionelles Patella-Malalignment zu erkennen bzw. dieses auszuschließen oder zu behandeln.

Summary: Patellofemoral pain (PFP) is a frequently observed pain phenomenon of multifactorial origin. Anatomical, functional, biomechanical and other factors play a role in its etiopathogenesis. The influence of social and psychological aspects remains unclear.

The diagnostics should include a detailed medical history and a thorough clinical examination in which functional tests and exercises are of decisive importance. In addition, musculoskeletal imaging should focus on the exclusion of intra-articular pathologies and on the assessment of an anatomical or functional patellar malalignment, with potential implication of treatment.

Dysphagie als Komplikation nach einer Operation an der Halswirbelsäule

Zusammenfassung:

Dysphagien (Schluckstörungen) sind die häufigste Komplikation nach Operationen an der Halswirbelsäule. Bislang gibt es kein valides und reliables klinisches Untersuchungsinstrument, um Patienten nach einer Operation an der Halswirbelsäule zuverlässig hinsichtlich bestehender Schluckstörungen zu detektieren. Dies ist jedoch erforderlich, um eine aussagekräftige instrumentelle Diagnostik und therapeutische Maßnahmen einleiten zu können. Im Rahmen dieses Artikels sollen neben einem strukturierten Überblick über die aktuelle Forschungslage Kooperationsprojekte zwischen Logopädie und Orthopädie am SRH Klinikum Karlsbad vorgestellt werden, in deren Rahmen unter anderem klinische Parameter ermitteln werden sollen, um Risikopatienten schnell und zuverlässig zu identifizieren.

Summary: Dysphagia (swallowing disorders) is the most common complication after cervical spine surgeries. So far, there is no valid and reliable clinical examination instrument to reliably detect patients with swallowing disorders after cervical spine surgery. However, this is necessary in order to be able to initiate instrumental diagnostics and therapeutic management. In the context of this article, a structured overview of the current research is given, followed by an introduction of cooperational projects between speech therapy and orthopedics at SRH Klinikum Karlsbad. The aim of these projects is for example to identify clinical parameters in order to select patients at risk of dysphagia quickly and reliably.

Multimodale Therapie bei chronischer Lumbago

Zusammenfassung: Chronische Lumbago ist im täglichen Patientengut ein sehr häufiges Krankheitsbild und führt nicht selten zu starken Alltagseinschränkungen der Patienten sowie zu hohen Kosten im Sozialsystem. Die zur Verfügung stehenden medikamentösen und interventionellen Therapien alleine weisen oftmals einen nur limitierten Effekt auf, weil es sich beim Krankheitsbild der chronischen Lumbago in der Regel nicht um eine monokausale Erkrankung handelt, sondern ein Syndrom mit organischen und psychischen Anteilen vorliegt. Um diesem vielschichtigen Krankheitsbild gerecht zu werden, werden interdisziplinäre und integrierte Therapien benötigt. Insbesondere bevor bei dieser Patientengruppe eine operative Maßnahme in Betracht gezogen wird, sollten konservative Maßnahmen einschließlich einer multimodalen Schmerztherapie erfolgt sein.

Summary: Chronic lower back pain is a very commonly seen affliction in our everyday patients, often associated with significant disability in daily activities, as well as high social costs. Available pharmacological and interventional therapies (as monotherapy) lead more often than not to only limited results. The reasons behind this concern the etiological ramifications of lower back pain, which should not be approached as a monocausal disease, but rather as complex syndrome of organic and psychological elements requiring integrative and interdisciplinary treatment. Before considering surgery, the patients should undergo an interdisciplinary multimodal pain therapy.

Management der akuten traumatischen cervikalen Querschnittlähmung

Zusammenfassung:

Patienten mit akuter traumatischer Verletzung des cervikalen Rückenmarks stellen eine komplexe klinische Herausforderung dar. Derartige Verletzungen führen meist zu motorischen und sensorischen Defiziten sowie vegetativen Störungen. Eine höhere Beachtung der intensivmedizinischen Betreuung hat bei vielen Patienten zu einer verbesserten Überlebenschance und Outcome geführt. Die Methoden und Technologien zur Diagnose und Klassifikation dieser Verletzungen haben sich in den letzten Jahrzehnten ebenso wie die medizinischen und chirurgischen Therapien erheblich weiterentwickelt. Unter Berücksichtigung der aktuellen Literatur sowie der eigenen Erfahrungen wollen wir die Optionen bezüglich der Diagnostik und Behandlung von Patienten mit akuter traumatischer Schädigung des cervikalen Myelons aufzeigen.

Summary: Patients with acute traumatic injury to the cervical spinal cord represent a complex clinical challenge. Such injuries usually lead to motor and sensory deficits as well as vegetative disorders. Increased attention to intensive care has led to improved survival and outcome for many patients. The methods and technologies used to diagnose and classify these injuries as well as medical and surgical therapies have developed considerably in recent decades. Under consideration of the current literature and our own experience, we want to show the options regarding the diagnosis and treatment of patients with acute traumatic damage to the cervical spinal cord.
