Anwendung von Ceracell Foam bei lumbalen Spondylodesen

Einleitung und Ziel: Bei einer Spondylodese werden häufig Knochenersatzstoffe als Fusionsmaterial eingesetzt. Das in dieser retrospektiven Beobachtungsstudie angewandte Ceracell Foam ist ein ?-Tricalciumphosphat (curasan AG, Kleinostheim). Ziel dieser Studie war die radiologische Beurteilung der Knochenneubildung und der gleichzeitigen Resorption des Ceracells.

Patienten und Methode: In diese retrospektive, nichtinvasive, offene, monozentrische Studie wurden 60 Patienten mit Indikation zur Spondylodese im Bereich der LWS eingeschlossen. Als Fusionsmaterial wurden autologer Knochen und das mit Knochenmark vermischte Ceracell Foam angewandt. Es erfolgten die radiologische Untersuchungen der Knochenneubildung und der Resorption des Keramikanteils direkt postoperativ, nach 3 Monaten und nach 12 Monaten.

Ergebnisse: Die Anwendung des Ceracell Foam während der Operation war stets technisch einfach. Komplikationen, die mit dem synthetischen Fusionsmaterial assoziiert werden können, lagen nicht vor. Bei 8 Patienten wurden postoperative Komplikationen gezählt: bei 3 Patienten trat eine Frühlockerung der eingebrachten Implantate auf, bei 1 Frühinfekt wurde eine Wundrevision bei 1 Patienten notwendig, bei 2 Patienten wurde eine Spätlockerung der Implantate gezählt, 2 Patienten gaben klinisch eine Ischialgie postoperativ bei erneutem Bandscheibenvorfall bzw. restlicher Spinalkanalstenose an. Eine operative Revision wurde bei 7 Patienten notwendig. Die Auswertung der radiologischen Bilder präsentierte im Verlauf eine deutliche Zunahme der Fusionsstrecke bei gleichzeitig zunehmender bildmorphologischer Resorption des Keramikanteils.

Schlussfolgerung: Das angewandte Ceracell als synthetisches Knochenmaterial zeigt in der vorliegenden Studie im Bereich der LWS gute Fusions-Eigenschaften. In Kombination mit autologem Knochen und Knochenmarkaspirat nahm die Fusion im postoperativen Verlauf sicher zu, bei gleichzeitiger Abnahme des Keramikanteils. Bei retrospektiver, rein deskriptiver Studie sind jedoch weitere Studien mit größeren Kollektiven mit prospektivem Verlauf und einer gleichzeitigen Untersuchung der postoperativen Klinik der Patienten zu planen. Ebenfalls sollten Vergleiche bezüglich der fusionsfördernden Eigenschaften zu weiteren Knochenersatzstoffen angestrebt werden.

Introduction and purpose: In spondylodesis bone substitutes are often used as fusion material. The Ceracell foam (curasan AG, Kleinostheim) used in this retrospective observational study is a beta-tricalcium phosphate. The aim of this study was the radiological assessment of the new bone formation and simultaneous examination of the resorption of the Ceracell.

Patients and methods: This retrospective, non-invasive, monocentric study included 60 patients with indication for spondylodesis in the lumbar spine. Autologous bone and Ceracell mixed with bone marrow were used as fusion material. Radiological examinations of new bone formation and resorption of the ceramic portion were performed immediately postoperatively, after 3 months and after 12 months.

Results: The use of the Ceracell foam during surgery was always technically simple. There were no complications that could be classified as associated with the fusion material. Eight patients showed postoperative complications: In 8 patients postoperative complications were counted: in 3 patients early loosening of the inserted implants occurred, in 1 patient an early infection required wound revision, in 2 patients a late loosening of the implants was counted, 2 patients clinically reported ischialgia postoperatively in case of a renewed herniated disc or residual spinal canal stenosis. A surgical revision was performed on 7 patients. The radiological evaluation showed a significant increase in the fusion distance with a simultaneous increase in the image morphological resorption of the ceramic portion.

Conclusion: In the present study, the applied Ceracell foam as a synthetic bone material showed good fusion properties in the area of the lumbar spine. In combination with autologous bone and bone marrow aspirate the fusion increased safely in the postoperative course. In the present retrospective, purely descriptive study, however, further studies with larger collectives and an examination of the postoperative clinic of the patients are to be planned. Comparisons should also be made regarding the fusion-promoting properties to other bone substitutes.
