Periprothetische Frakturen des distalen Femurs sind relativ seltene, aber komplexe Verletzungen. Der behandelnde Chirurg muss bei der Versorgung dieser Frakturen nicht nur mit den Techniken der Revisionsendoprothetik vertraut sein, sondern auch Erfahrungen in der osteosynthetischen Frakturbehandlung haben. Aufgrund der zunehmenden Lebenserwartung sowie einem hohen Aktivitätsniveau älterer Patienten ist mit einem Anstieg der Inzidenz periprothetischer Frakturen zu rechnen. Die aktuellen Behandlungsoptionen in Form von winkelstabilen Plattenosteosynthesen, Marknagelosteosynthesen sowie modularen Endoprothesensystemen haben die postoperativen Ergebnisse nach distalen periprothetischen Femurfrakturen verbessert.
Summary: Periprosthetic fractures of the distal femur are relatively rare but complex injuries. The treating surgeon should not only be experienced in revision arthroplasty, but also in osteosynthetic fracture treatment. Due to increasing life expectancy and a high level of activity of elderly patients, an increase in the incidence of periprosthetic fractures is expected. Current options to treat these fractures range from osteosynthesis using the locking-compression-plate of the distal femur or intramedullary nailing up to modular endoprosthesis systems. These techniques have helped to improve the postoperative results after distal periprosthetic femoral fractures.
