Anwendung von winkelstabilen Formplatten bei der Versorgung komplexer Gelenkfrakturen
Zusammenfassung: Komplexe Gelenkfrakturen gehen meist mit einem Weichteilschaden und ggf. der Verletzungen von Binnenstrukturen einher, was die operative Therapie zu einer Herausforderung macht. Besonders drastisch kann diese im Falle der unteren Extremität sein. Anatomisch geformte, winkelstabile Platten stellen den aktuellen Standard in der Therapie dar. Die exemplarische Darstellung der Versorgung komplexer Frakturen von distalem Femur und proximaler Tibia mit winkelstabiler Formplatte zeigt, dass hiermit gute funktionelle Ergebnisse und geringe Komplikationsraten erzielt werden können. Große Bedeutung kommt den Weichteilen zu – und in deren Rahmen den minimalinvasiven Operationstechniken. Die Prinzipien der Frakturversorgung, wie z.B. die dynamische Osteosynthese gelten Implantat-unabhängig und müssen konsequent angewendet werden.
Summary: Complex articular fractures are frequently associated with soft tissue damage, and concomitant intraarticular injuries which makes surgical treatment a challenge. This is of most relevance in the lower limb. Anatomical shaped locking plates are the current gold standard of therapy. Exemplary description of complex fracture treatment of the distal femur and the proximal tibia using anatomical locked plating shows that good functional results and low complication rates can be achieved. Soft tissue handling is crucial, i.e. minimal invasive techniques should be applied. Principles of fracture treatment, e.g. dynamic plating count regardless of the implant and need to be applied consequently.
