Knorpeltherapie im Patellofemoralgelenk
Zusammenfassung: Knorpelschäden des Kniegelenks bedeuten eine signifikante Einschränkung für den Betroffenen. Zudem erhöht sich die Gefahr der Entwicklung einer Arthrose deutlich. Aus diesem Grund gilt die Empfehlung, bei fokalen Schäden und Einhaltung der Indikationskriterien eine operative Intervention anzustreben. Aufgrund der anatomischen Komplexität dieses Gelenkabschnitts stellt die patellofemorale Knorpelchirurgie eine besondere Herausforderung dar. Co-Pathologien bestehen in einem sehr hohen Anteil der Betroffenen und müssen adressiert werden. Durch moderne Verfahren der Knorpeltherapie mit korrektem Angehen der Co-Pathologie lassen sich aber mittlerweile zufriedenstellende Ergebnisse erreichen. Jedoch reichen diese Ergebnisse meistens nicht an die des tibiofemoralen Gelenkabschnitts heran.
Summary: Symptomatic osteochondral lesions of the patellofemoral joint are clinically challenging to manage because of the limited healing potential of articular cartilage; the complex morphology of the patellofemoral joint; the heterogeneity of the articular surface between patients; and high stresses across the joint, which can be altered by malalignment, tilt or maltracking. Co-pathology has to be addressed whenever existent. Irrespective of the surgical technique used, outcomes are generally worse in the patellofemoral compartment than in the tibiofemoral joint. The concomitant management of associated pathology, including patellar malalignment, is recommended because it has been shown to improve the success of cartilage restoration procedures.
