Zusammenfassung: Frakturen des Radiuskopfs gehören zu den häufigsten Verletzungen des adulten Skeletts. Während nicht dislozierte Frakturen eindeutig konservativ und gering dislozierte Frakturen mit guten Ergebnissen durch eine offene Reposition und Osteosynthese behandelt werden können, ist die Therapie komplexer Radiuskopffrakturen mit osteoligamentären Begleitverletzung ausgesprochen anspruchsvoll. Der endoprothetische Ersatz des Radiuskopfs stellt hierbei einen Teilaspekt des möglichen therapeutischen Spektrums dar. Im Folgenden werden die anatomischen und biomechanischen Grundlagen vermittelt und die Indikationsstellung sowie die operative Technik dargestellt.
Summary: Fractures of the radial head are common in adult trauma. Simple fractures without dislocation and fractures with simple articular involvement can be treated easily non-surgical or with open reduction and osteosynthesis. However, the treatment of comminuted fractures with associated osseus and ligamentous lesions is still on debate. Radial head arthroplasty is considered to be a good treatment option, since clinical results are mostly good and predictable. This article highlights the anatomic and biomechanical backgrounds and discusses indications and the surgical technique.
