Diagnostik und Therapie der akuten Patellaluxation
Zusammenfassung: Die Luxation der Patella ist eine häufige Verletzung des Kniegelenks, insbesondere bei Jugendlichen und jungen Erwachsenen. Die Indikation zur konservativen und operativen Therapie sowie die Wahl der geeigneten Operationsmethode werden weiterhin kontrovers diskutiert. Die Bedeutung des MPFL für die mediale Stabilität der Patella ist bewiesen, die Problematik der Dysplasie der Trochlea femoris und deren Therapie wird zunehmend berücksichtigt und ein Therapieansatz, der sich an den pathologischen Veränderungen des Patellofemoralgelenks orientiert, ist zu favorisieren. Die laterale Retinakulumspaltung hat in der Behandlung der akuten Patellaluxation keinen Stellenwert mehr.

Summary: Patella dislocation is a common injury of the knee joint among adolescents and young adults. The indication for conservative or operative therapy is not clear; and which kind of operative technique for patella stabilization seems appropriate, is still under discussion. The importance of MPFL is proven, the problem of trochlea dysplasia and its therapy seems still unsolved. Nevertheless trochleaplasty is one of the options in operative therapy and the way of choosing the right therapy according to the complex pathologies is more and more accepted. Lateral release is no longer part of the therapy options in acute patella dislocation.
