Behandlungsstrategien und Komplikationsmanagement dorsaler Fusionen bei Osteoporose
Zusammenfassung: Osteoporose gehört zu den häufigsten Erkrankungen des höheren Lebensalters. Erstmanifestation sind häufig Wirbelkörperfrakturen, die nicht selten eine operative Stabilisierung benötigen. Aber auch degenerative Veränderungen an der Wirbelsäule nehmen mit höherem Patientenalter an Bedeutung zu und werden vor dem Hintergrund der stetig steigenden Lebenserwartung und dem Anspruch der Patienten zunehmend häufiger therapiebedürftig. Daher ist anzunehmen, dass wir uns künftig vermehrt mit den Problemen der dorsalen Instrumentierung bei Osteoporose beschäftigen müssen. Pedikelschraubenlockerungen und -ausrisse sowie Anschlussfrakturen mit konsekutiver Kyphosierung sind in diesem Zusammenhang besonders zu nennen. Ziel der vorliegenden Arbeit war es, einen Überblick über schraubenspezifische Einflussfaktoren sowie Techniken der Verankerung und Instrumentierung zu geben, um das Auftreten besagter Komplikationen zu vermeiden.
Abstract: Osteoporosis is one of the most common diseases of the elderly. Initial manifestations are often vertebral fractures, which require surgical stabilization. However, with increasing age degenerative changes of the spine could be observed more often, too. Due to constantly increasing patient age and the patients’ growing demands, many of them need surgical therapy. Therefore it seems necessary to assume, that in the future we have to deal more often with complications of dorsal instrumentation in case of osteoporosis. In this context screw loosening, screw pull-out as well as adjacent fractures with consecutive kyphosis have to be mentioned. The aim of the present work is to provide an overview of the different pedicle screw designs and associated surgical techniques of fixation and instrumentation in order to avoid the occurrence of aforementioned complications.
