Die Arthroskopie – Operationsroutine versus Routinefehler
Zusammenfassung: Auch eine Routineoperation wie die Kniegelenkarthroskopie, die deutschlandweit über 400.000 mal pro Jahr durchgeführt wird, birgt nicht unerhebliche Gefahren für das Auftreten von Behandlungsfehlern. Bei der Gutachterkommission der Ärztekammer Nordrhein sind in den Jahren 2010–2015 insgesamt 9380 Verfahren abgewickelt worden. 2925 Verfahren = 31,8% betrafen die Orthopädie und Unfallchirurgie, davon wiederum 309 = 10,6% arthroskopische Operationen am Kniegelenk.
Die Fehlerquote betrug 29,1%. Die Meniskusresektion allein weist eine Fehlerquote von 20,7% auf. Die Kreuzband-Rekonstruktion ist hingegen durch eine erschreckend hohe Fehlerquote belastet: bei 85 Verfahren wurde 42 mal ein Fehler erkannt, entsprechend einem Anteil von 49,4%. Andere arthroskopische Eingriffe wie z.B. bei Arthrofibrose, freiem Gelenkkörper u.a. wiesen eine Fehlerquote von 22,6% auf. Aufklärungsrügen sind in den letzten Jahren seltener geworden.
Die technisch anspruchsvollen Operationen bedürfen einer kritischen Überprüfung der Indikation, der Qualifikation des Teams und der OP-Ausstattung.
Summary: Also a routine operation as the arthroscopy of the knee-joint, which is performed more than 400.000 times a year in Germany, contains the risk for medical malpractice. The committee of arbitration at the physician medical association in NRW (Gutachterkommission der Ärztekammer Nordrhein) handled 9380 proceedings from 2010–2015. 2925 cases (31.8%) were performed within Orthopedics and Traumatology. 309 of these cases = 10,6% were such with arthroscopic operative procedures. The rate of malpractice came to 29.1% over all, for the meniscus resection only 20.7%. In contrast the operative reconstruction of the anterior cruciate- ligament achieved failure rate of 49.4% in 42 patients among 85. Other arthroscopic procedures showed less malpractice with 22.6%. Complaining insufficient enlightenment or information became less important during the last years.
Especially the transplantation of the cruciate ligament, undergoing difficult anatomical orientation, requires a long experience for the indication and the technical practice, performing this operation.
