Zusammenfassung:Der direkte vordere Zugang (direct anterior approach/DAA) zum Einbringen einer primären Hüftendoprothese erlaubt eine muskelschonende Darstellung des Hüftgelenkes mit geringem Blutverlust, präziser Implantatpositionierung, guter Verankerung und schneller postoperativer Mobilisation in Vollbelastung. Nachteile sind die aufwändige Lagerung, die Lernkurve bei der Präparation und Implantatpositionierung und auch, dass man bei nennenswerten Schaftkomplikationen eingeschränkt handlungsfähig sein kann und der Zugang ausgeweitet werden muss. Auch ist der DAA bei der Schaft-Revisionsendoprothetik sicher nicht der Zugang der Wahl.Bei Beachtung einer standardisierten und sorgfältigen Präparation sind dem Zugang nachgesagte Komplikationen wie Muskel- und Nervenschäden, gut zu vermeiden. Postoperative Muskelinsuffizienzen treten kaum auf und Luxationen sind eine Rarität. Die kleine, kosmetisch gut gelegene Narbe, aber insbesondere die schnelle Mobilisation, führen zu einer hohen Zufriedenheit des Patienten.
Summary: The direct anterior appproach (DAA) is an excellent muscle sparing approach for hip joint replacement, associated with little blood loss, precise positioning and firm anchoring of implants, as well as swift recovery under full weight bearing. Disadvantages lie in the necessity for lengthy and precise patient positioning, a marked learning curve for the procedure itself and the fact that in some stem associated intraoperative complications solutions can be tedious. In our experience, the DAA is not the approach of choice for stem revision surgery.By following a very standardized and meticulous preparation, reported complications associated with this approach, such as damage to muscle tissue, trochanteric fractures and nerve irritations can usually be avoided. Postoperative muscle paralysis as well as luxations are extremely rare. The small, cosmetically hardly disturbing scar but more importantly, the markedly expedited recovery after surgery, lead to a high level of satisfaction in patients.
