Patellofemorales Schmerzsyndrom
Zusammenfassung: Das komplex aufgebaute Patellofemoralgelenk ist durch anatomische, biomechanische und neuromuskuläre Faktoren störanfällig für ein Maltracking der Patella mit resultierenden unphysiologischen Belastungen. Diese führen über osteochondrale und ligamentäre Veränderungen zu Schmerzen. Eine ursachenorientierte Klassifikation basiert auf einer gezielten Diagnostik und dient als Basis für eine differenzierte Therapie. Bei fehlenden strukturellen Schäden ist die konservative multimodale Therapie mit dem Fokus auf die Normalisierung der gestörten neuromuskulären Situation Mittel der Wahl. Nur bei strukturellen Schädigungen oder ausgeprägten Veränderungen der anatomischen Einflussfaktoren sollten chirurgische Maßnahmen in Betracht gezogen werden.
Summary: Based on anatomical, biomechanical and neuromuscular factors, the complex patellofemoral joint is susceptible to patellar maltracking with resulting unphysiological loading. Osteochondral and ligamentous changes lead to a pain syndrome. A cause-orientated classification is based on systematic diagnostics and serves as basis for a differentiated treatment. In the absence of structural damage, conservative multimodal treatment with special focus on normalizing the disturbed neuromuscular situation is the method of choice. Surgical procedures should only be considered in case of structural damage or pronounced alteration of anatomic factors.
