Die Endoprothetik des Handgelenks – ein positiver Ausblick
Zusammenfassung: Mit der Entwicklung der 4. Generation von Handgelenk-Implantaten kann bei geeigneter Indikation gegenüber der Arthrodese ein Zugewinn an Komfort und Funktion bei gleichzeitiger Schmerzreduktion/befreiung erreicht werden. Insbesondere RA-Patienten mit kontralateral bereits versteiften Gelenken bevorzugen die „bewegliche Lösung“, wenngleich der Bewegungs- und Kraftzugewinn nur moderat ausfällt. Wesentliche Komplikationsunterschiede zwischen degenerativen (OA) und entzündlichen Verlaufsformen (RA) lassen sich nicht nachweisen. Neuere Arbeiten zeigen günstigere Standzeiten der optimierten Prothesensysteme bei allerdings weiterhin kleinen Fallzahlen und kurzen Nachuntersuchungszeiträumen. Junge Patienten, ein hoher Belastungsanspruch, insuffizienter Knochenstatus oder erhebliche radiokarpale Instabilitäten stellen weiterhin eine Kontraindikation dar.
Summary: With the development of the 4th generation of wrist implants, an improvement in comfort and function while at the same time relieving pain can be achieved with suitable indication against the arthrodesis. In particular, RA patients with contralateral stiffened joints prefer the „mobile solution“, although the movement and power gain is only moderate. Significant complication differences between degenerative (OA) and inflammatory progression forms (RA) cannot be demonstrated. Recent studies show more favorable outcomes and implant survival of the optimized prosthetic systems, but still small case numbers and short follow-up periods underline the necessity of long-term data. Young patients, a high stress claim, insufficient bone status or significant radiocarpal instabilities continue to be a contraindication. Additional prospective studies are needed to compare outcomes of wrist arthrodesis with those of TWA with current implants.
