Zusammenfassung: Die patellofemorale Instabilität stellt ein häufiges und zugleich komplexes Krankheitsbild dar. Primäres Ziel der Therapie ist das Verhindern weiterer Luxationsereignisse. Bei einer erstmaligen Luxation der Patella ohne relevante Begleitverletzungen und Risikofaktoren besitzt die konservative Therapie weiterhin einen relevanten Stellenwert. Jedoch ist eine zielgerichtete klinische und radiologische Diagnostik entscheidend, um vorliegende Risikofaktoren zu identifizieren und bestmöglich zu therapieren. Die operative Rekonstruktion des medialen patellofemoralen Ligaments (MPFL) hat sich als Basistherapie etabliert. Bei Vorliegen relevanter anatomischer knöcherner Risikofaktoren wie einer Trochleadysplasie, einer Patella alta, einer pathologischen Lateralisation der Tuberositas tibiae sowie Torsions- und Achsfehlstellungen reicht eine isolierte MPFL-Plastik jedoch häufig nicht aus. In diesen Fällen sind knöcherne Korrekturen essenzieller Bestandteil eines erfolgreichen Therapiekonzepts.
Summary: Patellofemoral instability is a common yet complex clinical condition. The primary goal of treatment is to prevent recurrent dislocation events. Following a first-time patellar dislocation without relevant concomitant injuries or significant risk factors, conservative management continues to play an important role. However, focused clinical and radiological assessment is essential to identify underlying risk factors and to guide optimal treatment. Surgical reconstruction of the medial patellofemoral ligament (MPFL) has become the established cornerstone of surgical treatment. Nevertheless, in the presence of relevant osseous anatomical risk factors such as trochlear dysplasia, patella alta, pathological lateralization of the tibial tubercle, as well as torsional and axial malalignment, isolated MPFL reconstruction is often insufficient. In such cases, bony realignment procedures are an essential component of a successful treatment strategy.
