Operative Therapie von Patella-Frakturen
Zusammenfassung: Frakturen der Patella sind eine seltene Entität, deren Versorgung relativ komplex sein kann. Zur Bestimmung der exakten Frakturmorphologie und zur Operationsplanung ist eine Computertomographie empfehlenswert. Frakturen mit einer Dislokation über 2mm sowie Mehrfragmentfrakturen, offene Frakturen oder osteochondrale Frakturen sollten operativ behandelt werden. Zur operativen Versorgung stehen mehrere Osteosyntheseverfahren zur Auswahl. Die klassische Zuggurtung ist nach wie vor am meisten genutzt, wenn auch moderne winkelstabile Plattensysteme bessere biomechanische Ergebnisse zeigen konnten. Für Querfrakturen eignet sich bei guter Knochenqualität die kanülierte Schraubenosteosynthese, die mit einer Zuggurtung kombinierbar ist. Trotz einer zufriedenstellenden Reposition sind die Langzeitergebnisse, bezogen auf die Patientenzufriedenheit, als mittelmäßig einzustufen.
Summary: Only 1% of all fractures happen to the patella. The treatment of patella fractures can be very complex and difficult. For the exact fracture mapping and the ideal preoperative planning a CT-scan should be obtained. Fractures with a dislocation of more than 2mm, multifragment fractures, open fractures or osteochondral fractures should be treated operatively. There are different types of osteosynthesis to choose from. The traditional tension wire osteosynthesis is the most popular, even though modern locking plates turned out to be biomechanically superior. Transverse fractures of the patella can be treated with cannulated screws in combination with tension band wiring. Despite good reduction and reposition of the fracture many patients show persistent complaints in the long-term subjective postoperative outcome.
