Vertiefende Trochleaplastik – Indikation, Technik, Ergebnisse
Zusammenfassung: Die vertiefende Trochleaplastik stellt ein Standardverfahren in der Behandlung der patellofemoralen Instabilität dar. Operationsziel ist die stabile Rezentrierung der Patella durch Normalisierung der Eingangsebene bei Vorliegen einer Trochleadysplasie. Die Indikation besteht bei nachgewiesener patellofemoraler Instabilität mit rezidivierenden Luxationen der Patella und Vorliegen einer hochgradigen Trochleadysplasie mit konvexer Eingangsebene. Die Trochleaplastik kann mit möglicherweise notwendigen Zusatzeingriffen wie der Rekonstruktion des medialen patellofemoralen Ligaments oder einer Medialisierung der Tuberositas tibiae kombiniert werden. Kontraindikationen sind offene Wachstumsfugen am distalen Femur sowie höhergradige Knorpelschäden an der Trochlea femoris. Mehrere Studien belegen den Erfolg der operativen Maßnahme mit guten klinischen Ergebnissen und hoher Patientenzufriedenheit.
Summary: Deepening trochleoplasty is a surgical procedure, which is able to reshape the dysplastic femoral trochlea in patients with patellofemoral instability. The procedure is of paramount importance, because trochlear dysplasia is the most important risk factor for patellofemoral instability. The ideal indication for trochleoplasty is documented patellofemoral instability and high-grade trochlear dysplasia. Contraindications are open distal femoral physis and degenerative changes of the patellofemoral joint. Deepening trochleoplasty can be combined with reconstruction of the MPFL and medialisation of the tibial tubercle, if necessary. Very encouraging results have been reported by different authors from the application of deepening trochleoplasty combined with other procedures such as MPFL reconstruction.
