Der Pararectus-Zugang – Innovation in der Azetabulumchirurgie
Zusammenfassung: Während über Jahrzehnte der ilioinguinale Zugang zur ventralen Versorgung von Azetabulumfrakturen verwendet wurde, haben aufgrund des beim alten Menschen veränderten Frakturmusters die intrapelvinen Zugänge, der modifizierte Stoppa-Zugang und der Pararectus-Zugang an Bedeutung gewonnen. Der Pararectus-Zugang verbindet die Vorteile der medialen Sicht des Stoppa- und des zweiten Fensters des ilioinguinalen Zugangs in einem Zugang. In dieser Übersichtsarbeit beschreibt der Erfinder des Zugangs die operative Technik, Vor- und Nachteile und die ersten mittelfristigen Resultate, sowie der erste internationale „Pararectus-Schüler“ seine Argumente für den Zugang.
Summary: During the last decades the ilioinguinal approach was the gold standard for the operative treatment of acetabular fractures involving predominantly the anterior column. Due to the new fracture patterns in elderly patients, the intrapelvic approaches, the modified Stoppa and the Pararectus approaches are becoming more popular. The Pararectus approach combines the advantages of the medial view through the modified Stoppa and the second window of ilioinguinal approach in one access. In this review article the inventor of this approach describes the operative technique, the pros and cons and the first midterm results, as well as the first international „Pararectus student“ gives his arguments for this approach.
