Zusammenfassung: Der Beitrag stellt die Vorgehensweise bei der Indikationsprüfung einer OSG-Prothese dar. Des Weiteren werden das operative Verfahren, die Nachbehandlung und das Komplikationsmanagement dargestellt. Die Ergebnisdarstellung erfolgt mittels des etablierten deutschen OSG-Protehseregisters der Deutschen Assoziation für Fuß und Sprunggelenk (DAF). Eine prothetische Versorgung des OSG beinhaltet nur einen Ersatz der destruierten Gelenkflächen. Bestehen periartikuläre Probleme, Fehlstellungen und Instabilitäten, so müssen diese zusätzlich korrigiert werden.
Summary: Total joint replacement has evolved as valuable treatment option in end stage ankle arthritis. Indication, operative procedures and postoperative management is discussed with special emphasis on strategies to avoid typical complications. Additional procedures to establish a balanced alignment of the ankle joint may be needed to ensure lasting and pain free function. The total ankle joint arthroplasty registry of the German Orthopedic Foot and Ankle Society (DAF) has been introduced to evaluate survival rates of various implants. Preliminary Data from this national registry show an increased benefit of patients with rheumatoid arthritis after total ankle replacement.
