Tendinitis calcarea

Zusammenfassung: Die Kalkschulter (Tendinitis calcarea) ist ein häufiges und schmerzhaftes Krankheitsbild der Schulter mit großer Relevanz im klinischen Alltag des Orthopäden. Der mitunter langjährige Krankheitsverlauf ist in der Regel gekennzeichnet durch eine phasenhafte Entwicklung des Kalkdepots. Symptomatisch werden die meist zwischen 30 und 50 Jahre alten Patienten durch Schmerzen, die ohne Provokationsereignis auftreten und bewegungsunabhängig sind. Die klinische Symptomatik gleicht überwiegend einem Impingement-Syndrom mit Ruhe- und Nachtschmerz und Schmerzausstrahlung in den anterolateralen Oberarm. Im Rahmen einer akuten Exazerbation, die mit der spontanen Kalkresorption assoziiert ist, zeigen die Beschwerden mitunter einen massiven entzündlichen Charakter.

Konservative Maßnahmen umfassen die Gabe von Analgetika und Antiphlogistika, subakromiale Infiltrationen und extrakorporale Stoßwellentherapie. Bei Beschwerdepersistenz ist die arthroskopische Kalkentfernung indiziert, die in 90% der Fälle zu Beschwerdefreiheit und guten bis sehr guten klinischen Ergebnissen führt.

Abstract: Rotator cuff calcific tendinitis is a frequent and painful clinical entity with great relevance to the orthopaedic surgeon. The disease history may be one of many years and is characterised by distinct phases in the development of the calcific deposit. Mainly affecting patients between 30 and 50 years, the disease becomes symptomatic in terms of unprovoked pain unrelated to activity. The clinical presentation is similar to the impingement syndrome with pain at rest and at night, which may radiate to the anterolateral proximal arm. The acute exacerbation is marked by inflammatory changes and may be associated with resorption of the calcific deposit. Conservative management includes analgesic and anti-inflammatory drugs, subacromial corticoid injections and extracorporal shock wave therapy. If this approach fails and patients are persistently symptomatic, arthroscopic removal of the calcific deposit is indicated. In more than 90% of patients the latter management is definite and provides good-to-excellent clinical outcomes.
