Das schwerste Kapitel: Kodieren von Komplikationsdiagnosen in Orthopädie und Traumatologie
Zusammenfassung: In einem Aufsatz im Jahr 2009 in der Orthopädischen Praxis [1] wurde eine Einführung in die Diagnose- und Therapiekodierung nach den Deutschen Kodierrichtlinien publiziert. Der Bericht wird nun mit dem problematischsten Teil der Kodierung fortgesetzt – der Kodierung von Komplikationsdiagnosen. Am problematischsten stellt sich die Kodierung von Komplikationen dar, weil Krankenhäuser und Kostenträger trotz eindeutiger Kodierrichtlinien hier oft unterschiedliche Meinungen vertreten. Nach allgemeinen Bemerkungen zum Thema wird die Definition einer Komplikation wiederholt und die ganze Bandbreite der Komplikationskodierung in der Orthopädie und Traumatologie an Hand konstruierter Beispieldatensätze dargestellt.
Summary: In a survey in 2009 in the journal “Orthopädische Praxis” an introduction in the coding of diagnosis and procedures according to Diagnose related groups (DRG) was published [1]. This survey now will be continued with the most problematic part of coding, the coding of diagnosis of complications. The most problematic coding is caused by different interpretations of the codes between the hospitals and the health insurance, although the DRG are unmistakable. After some general remarks and a definition of complications, the whole spectrum of coding of complications in orthopaedics and traumatology will be given by constructed examples.
