Zusammenfassung: Proximale Humerusfrakturen zählen zu den häufigsten Frakturen des höheren Lebensalters und entstehen überwiegend durch Niedrigenergietraumata wie Stürze auf den ausgestreckten Arm [1, 2]. Die operative Versorgung mittels winkelstabiler Plattenosteosynthese, intramedullärer Nagelung oder minimal-invasiver Verfahren ist insbesondere bei dislozierten oder komplexen Mehrfragmentfrakturen indiziert [3, 4]. Trotz moderner Implantate und verbesserter Operationstechniken bleibt die Komplikationsrate relevant und umfasst unter anderem Schraubenperforationen, sekundäre Dislokationen, avaskuläre Nekrosen, Pseudarthrosen, Infektionen und postoperative Schultersteife [5, 6]. Das vorliegende Manuskript gibt einen strukturierten Überblick über aktuelle Klassifikationen, Risikofaktoren, typische Komplikationen und evidenzbasierte Revisionsstrategien. Darüber hinaus werden Unterschiede zwischen den gängigen Osteosyntheseverfahren dargestellt und klinisch relevante Handlungsempfehlungen für Diagnostik und Therapie abgeleitet.
Summary: Proximal humerus fractures are among the most common fractures in older adults and typically result from low-energy trauma such as falls onto the outstretched arm [1,2]. Surgical treatment using locking plate fixation, intramedullary nailing, or minimally invasive techniques is particularly indicated for displaced or complex multi-fragment fractures [3,4]. Despite advances in implant design and surgical technique, complication rates remain clinically relevant and include screw perforation, secondary displacement, avascular necrosis, nonunion, infection, and postoperative stiffness [5, 6]. This manuscript provides a structured overview of current classifications, risk factors, common complications, and evidence-based revision strategies. It further outlines the differences between established osteosynthesis techniques and derives clinically relevant recommendations for diagnostic and therapeutic decision-making.
Zusammenfassung: Winkelstabile Systeme haben die Behandlung und das klinische Outcome von osteoporotischen Frakturen in den letzten Jahren nachhaltig verbessert. Bei der operativen Versorgung von proximalen Humerusfrakturen stellen die hohen postoperativen Komplikationsraten weiterhin ein alltägliches Problem dar. Als häufige Komplikationen werden sekundäre Dislokationen, intraartikuläre Schraubenlagen, Humeruskopfnekrose und Rotatorenmanschetteninsuffizienz beschrieben. Die Verwendung moderner winkelstabiler Systeme mit der Möglichkeit der Zementaugmentation, die zusätzliche Fixierung der Rotatorenmanschette mittels Fadencerclagen und biomechanische Erkenntnisse helfen, die Komplikationsraten zu verringern und damit das klinische Ergebnis zu verbessern. Der vorliegende Artikel legt die aktuellen Therapiekonzepte bei der Versorgung der osteoporotischen proximalen Humerusfraktur dar, zeigt die Grenzen der Rekonstruierbarkeit auf und gibt einen kurzen Ausblick auf die Neuerungen bei der Versorgung osteoporotischer proximaler Humerusfrakturen.
Summary: Locking systems have dramatically improved the treatment of osteoporotic fractures during the last decade. However, the clinical outcome of proximal humeral head fractures is still often accompanied by complications. Most common problems are secondary dislocation, screw perforation, avascular head necrosis and secondary insufficiency of the rotator cuff. The usage of modern locking systems with the ability of cement augmentation of the locking screws, the additional fixation of the rotator cuff with sutures and biomechanical knowledge can help to reduce the complication rates and can improve the clinical outcome. This article provides the current treatment options in the therapy of humeral head fractures in osteoporotic bone. Futhermore, the current developments are highlighted together with the limitations of the locking systems.
