Chronische Rückenschmerzen – entzündlich, funktionell, psychosomatisch?
Zusammenfassung: Chronische Rückenschmerzen sind in der westlichen Hemisphäre ein wesentliches medizinisches Problem. Morphologische, somatisch funktionelle und psychosoziale Aspekte sowie Mechanismen der neurophysiologischen Schmerzchronifizierung sind an der Pathogenese beteiligt. Schon im akuten Stadium sollte ein Screening auf Risiken für eine Schmerzchronifizierung erfolgen. Patienten mit chronischen, rezidivierenden oder chronifizierungsgefährdeten Rückenschmerzen bedürfen einer multimodalen interdisziplinären Diagnostik.
Im Rahmen der Diagnostik können die wesentlichen morphologischen, somatisch funktionellen, psychosozialen und neurophysiologischen Aspekte der Schmerzchronifizierung erhoben und bewertet werten. Auf dieser Grundlage lässt sich ein befundgerechtes mono- oder multimodales Therapiekonzept erarbeiten. In den Rückenzentren und stationär im Rahmen der multimodalen interdisziplinären Komplexbehandlung des Bewegungssystems (ANOA-Konzept) werden solche Diagnostik- und Therapieansätze erfolgreich umgesetzt.
Summary: Chronic low back pain (cLBP) is one of the major medical problems in the western world. Morphological and psychosocial aspects, somatic dysfunction and neurophysiological mechanism of pain chronification are important for the development of cLBP. Even in patients with acute LBP the diagnostic procedure should be well structured. Patients with a chronic or recurrent LBP or with risk factors for the development of a cLBP should undergo a multimodal interdisciplinary diagnostic in order to evaluate morphological, psychosocial, functional and neurophysiological findings. Following the diagnostic, an individual mono- or multimodal treatment program can be developped for each patient.
These diagnostic and treatment strategies are daily praxis in the ”German Spine Centers” and in the context of the multimodal interdisciplinary treatment concept of the locomotor system (ANOA-Concept).
