Zusammenfassung: Das Hauptziel der interventionellen Wirbelsäulentherapie ist es, die schmerzgeplagten Rückenpatientinnen und -patienten, die möglicherweise sogar vor einem operativen Eingriff stehen, über deren Schmerzspitzen hinwegzuhelfen. Spinalnervanalgesien, epidurale Injektionen, Facettengelenk- und ISG-Infiltrationen wirken direkt am Ausgangspunkt der Nozizeption im Bewegungssegment und führen sehr häufig maßgeblich zu einer Befundbesserung. Diese Injektionen lassen sich sicher und effektiv anhand anatomischer Landmarken durchführen. Dies geschieht vor allem strahlenfrei, ohne kostenintensiven apparativen Einsatz und mit einem geringen organisatorischen und zeitlichen Aufwand.
Summary: The main goal of interventional spine treatment is to help back pain patients, who may even think about surgery, get over their pain peaks. Nerve root blocks, epidural injections, facet injections and sacroiliac joint injections are procedures that influence the pathological process directly in the spine motion segment and very often lead to a significant improvement in the findings. These spinal injections can be performed safely and effectively using anatomical landmarks. Above all, this is done radiation free, without the use of expensive equipment and with low organizational effort and time.

Spinalnervanalgesien, epidurale Injektionen, Facettengelenk- und ISG-Infiltrationen wirken direkt am Ausgangspunkt der Nozizeption im Bewegungssegment und führen häufig zu der „entscheidenden“ Befundbesserung schmerzgeplagter Rückenpatienten, die möglicherweise sogar vor einem operativen Eingriff stehen. Diese Injektionen lassen sich sicher und effektiv anhand topografisch anatomischer Orientierungspunkte durchführen. Dies geschieht vor allem strahlenfrei, ohne kostenintensiven apparativen Einsatz und mit einem geringen organisatorischen und zeitlichen Aufwand. Der vorliegende Beitrag gibt eine Übersicht über die wichtigsten neuroanatomischen Landmarken und vermittelt praxisnahe Tipps und Tricks zur sicheren und effizienten Durchführung der landmarkengestützten Injektionstechniken an der Wirbelsäule.

Summary: Nerve root blocks, epidural injections, facet and sacroiliac joint injections are procedures that influence the pathological process directly in the spine motion segment. They often lead to the decisive improvement of back pain patients, who may even think about a surgery. It is possible to perform these spinal injections considering only topographic anatomical landmarks. That happens without ionizing radiation and expensive equipment and with a low administrative and low time effort. The present article gives an overview on the important topographic anatomical landmarks and presents practically oriented tips and tricks for an efficient and safe performance of spinal injections without imaging.
