Aktueller Stand der Therapie des Morbus Perthes

Zusammenfassung: Die zweithäufigste Diagnose kindlicher Hüftgelenkerkrankungen betrifft den Morbus Perthes. Auch bis heute ist die Ätiologie der aseptischen Hüftkopfnekrose im Wachstumsalter nicht eindeutig geklärt.

Der Verlauf und die Prognose der Erkrankung ist sehr variabel und vom Ausgangsbefund und Alter und somit auch der Therapie der kleinen Patienten bei Erstdiagnose abhängig. Die Erkrankungsdauer erstreckt sich über mehrere Jahre und kann in einer Restitutio ad integrum oder aber in einer schweren Deformierung des Hüftkopfs enden. Das therapeutische Ziel ist heute ein optimales Containment von Hüftkopf und Hüftpfanne. So entwickelte sich die konservative und operative Therapie des M. Perthes in den letzten Jahrzehnten weg von abduzierenden und entlastenden Orthesen hin zu operativen gelenkzentrierenden Osteotomien, welche heute eine wesentliche Säule der Therapie sind.

Summary: The second most common diagnosis of childhood hip disease affects the Perthes disease. Even today, the etiology of aseptic osteonecrosis in growing children is not clear.

The course and prognosis of the disease is highly variable and on the initial findings and age, and thus the treatment of young patients at diagnosis dependent. The duration of illness is extending over several years and can result in a restitutio ad integrum or in a severe deformity of the femoral head. The therapeutic goal today is an optimal containment of the femoral head and acetabulum. Thus, the conservative and surgical treatment of Perthes’ disease joint developed in recent decades away from abducting and relieving orthoses towards operative centering osteotomies, which are essential components of therapy today.

Schmerzhafte Einschränkung der Unterarmdrehung durch das gestörte distale Radioulnargelenk nach Radiusfraktur

Zusammenfassung: Die schmerzhafte Einschränkung der Unterarmdrehung durch das gestörte distale Radioulnargelenk (DRUG) ist meist Folge einer Radiusfraktur, sei es, dass das DRUG unmittelbar in die Fraktur mit einbezogen ist oder auch durch eine Zerstörung der radioulnaren Bandverbindung. Bei der Primärversorgung einer frischen Fraktur sollte eine anatomische Rekonstruktion des Radius mit stufenfreier Wiederherstellung der Gelenkflächen im Radiokarpal- wie auch Radioulnargelenk erreicht werden. Bei der fehlverheilten Radiusfraktur ist das Radioulnargelenk im Prinzip intakt, die Funktionseinschränkung resultiert aus der Inkongruenz des Ellenkopfs mit dem fehlgestellten Radius. Zusätzlich führt die Fehlstellung wie auch ein Zusammensintern des Radius zu einer meist deutlichen Verkürzung desselben mit daraus resultierendem Ellenvorschub mit Ulna-Impaction-Syndrom.

Die Korrekturosteotomie des Radius stellt die Kongruenz des Radioulnargelenks wieder her, durch Beseitigung der Achsenfehlstellung einerseits, andererseits auch durch Erreichen der korrekten Höhe des Radius und beseitigt somit auch das Ulna-Impaction-Syndrom. Hier kann in den meisten Fällen eine restitutio ad integrum mit ungestörter Rotation erreicht werden.

Finden wir hingegen eine Arthrose im Radioulnargelenk oder auch eine veraltete Bandruptur radioulnar, so kann man die Problematik des Radioulnargelenks durch eine Radiuskorrektur nicht beseitigen. Hier sind dann Rettungsoperationen (z.B. radioulnare Arthrodese nach Kapandji, Hemiresektionsarthroplastik des Ellenkopfs nach Bowers oder auch Ulnarkopfprothese) indiziert.

Abstract: The most important reason of painful restricted forearm rotation results from fracture of the distal radius combined with restriction of the distal radioulnar joint (DRUJ). In case of obsolete dislocation of radial fracture, the DRUJ can be reconstructed by reconstructive osteotomy of the radius. It aims at recovering the normal position and simultaneous strengthening of the radius to eliminate incongruence of the DRUJ and simultaneously ulnar-impaction-syndrome.

In case of destroyed DRUJ, osteoarthritis or obsolete rupture of the radioulnar joint, the restoration of the joint is not possible, only a salvage procedure (Kapandji, Bowers or ulnar head prosthesis) can restore the function of DRUJ.

Analyse von Wechseloperationen bei Knietotalendoprothese

Zusammenfassung: Die vorliegende retrospektive Analyse von Wechseloperationen bei Knietotalendoprothese zeigt Unterschiede zwischen frühem und spätem Prothesenversagen. Die Indikation für einen Prothesenwechsel in der Gruppe der Frühversager bestand am häufigsten aufgrund einer Malrotation/Malposition der Prothesenkomponenten. Ein spätes Versagen war meist auf aseptische Lockerungen zurückzuführen. Mehrfach operierte Patienten erreichten in der Auswertung der Scores signifikant schlechtere Ergebnisse. Resultate im WOMAC und in Subskalen des SF-36 waren bei Patienten unter 49 und über 70 Jahren signifikant besser. Frauen und Patienten mit einem BMI 30 schnitten, ohne statistische Signifikanz, schlechter ab.

Abstract: The current retrospective analysis of revision surgery for total knee arthroplasty shows differences between early and late prosthetic failure. The indication for a replacement of the prosthesis in the group of early failure was found most frequently due to malrotation/malposition of the components. In contrast, late failure was mostly due to aseptic loosening. Patients that were treated with more than one surgical intervention reached significantly worse results in the used scores. Results in the WOMAC subscales and the SF-36 in patients under 49 and more than 70 years of age were significantly better. Women and patients with a BMI 30 performed worse, but without statistical significance.

Implantatfreie, anatomische MPFL-Plastik

Zusammenfassung: Zur Rekonstruktion des medialen patellofemoralen Ligaments (MPFL) werden eine Vielzahl an Techniken beschrieben, die typischerweise recht unterschiedliche Verankerungsmaterialien verwenden. Als Komplikationen werden Implantatlockerungen und/oder Frakturen mit oder ohne Luxationsrezidiv, ein chronisches anteriores bzw. ein antero-mediales Schmerzsyndrom und/oder ein anhaltendes Bewegungsdefizit beschrieben. Neben Problemen mit der Tunnelplatzierung wird eine fehlerhafte Transplantatspannung als Ursache diskutiert. Unsere Technik der implantatfreien, anatomischen Rekonstruktion erlaubt eine einfache intraoperative Austestung der Graftspannung beim Durchbewegen des Gelenks. Hierdurch hoffen wir, das Risiko einer Überspannung mit den entsprechenden Komplikationen zu reduzieren. In dieser Studie wurden 16 fortlaufende Patienten im Mittel 23 Monate postoperativ untersucht. Neben einer standardisierten klinischen Untersuchung wurde der Kujala-Score sowie ein Fragebogen zur Patientenzufriedenheit und zu den Möglichkeiten einer Rückkehr in den Sport verwendet. Der Kujala-Score zeigte eine signifikante Verbesserung mit guten Ergebnissen im Vergleich zur Literatur. Auch die Häufigkeit von Komplikationen war vergleichsweise niedrig. Entsprechend dieser Ergebnisse war auch die Patientenzufriedenheit in unserem Kollektiv als hoch anzusehen, sodass z.B. alle Patienten in die gleiche Sportart und, sofern dies zuvor kompetitiv betrieben wurde, auch in den gleichen Wettkampfsport zurückkehren konnten.

Abstract: For the reconstruction of the medial patellofemoral ligament (MPFL) a large number of procedures have been described which typically use diverse anchoring materials. Frequently described complications are implant loosening with or without fractures, recurrent luxation, and a chronic anterior or antero-medial pain syndrome, with or without lasting deficits in knee motion. Besides difficulties with correct tunnel placement, an incorrect graft tensioning is discussed as a reason for these complications. Our technique of an implant-free, anatomical reconstruction allows a simple intra-operative testing of the graft tensioning while the knee is moved. Using this technique, we expect to reduce the risks of an overconstraint of the graft with its corresponding complications. In this study, 16 consecutive patients were evaluated after a mean post-operative follow-up of 23 months. Besides a standardized clinical examination, the Kujala score and a questionnaire were used to evaluate patient satisfaction as well as the patients’ return to sports. The Kujala score showed a significant improvement with good results compared to the literature. Complications were also noted with a relatively low incidence. According to these results, a correspondingly high level of patient satisfaction was noticed, so that all patients were able to return to the same type of sport, and if it was done before, to the same competitive sport.
