Ballonkyphoplastie und konservative Therapie bei traumatischen Frakturen der Brust- und Lendenwirbelsäule

Zusammenfassung: Die Ballonkyphoplastie ist eine effiziente Therapieoption zur Behandlung von Wirbelkörperfrakturen. Sie ermöglicht eine schnelle Frakturstabilisierung und eine rasche postoperative Schmerzreduktion. Alternativ zur Ballonkyphoplastie besteht das Konzept der konservativen Therapie. Ziel dieser Arbeit war der Vergleich des Therapieerfolgs zwischen Ballonkyphoplastie und konservativer Therapie in der Behandlung von frischen, stabilen, traumatischen Wirbelkörperfrakturen der Brust- und Lendenwirbelsäule.

Aus den Ergebnissen kann gefolgert werden, dass im untersuchten Patientengut das Outcome durch die Ballonkyphoplastie dem der konservativen Therapie überlegen ist. Dies zeigt sich an einem höheren Körperfunktionsstatus in den Aktivitäten des täglichen Lebens sowie einer geringeren Schmerzintensität und kann durch die Verbesserung objektiver radiomorphologischer Parameter zusätzlich belegt werden.

Summary: Balloon kyphoplasty is an effective therapy option for the treatment of vertebral body fractures. It enables rapid fracture stabilization and rapid postoperative pain reduction. Alternatively, there is the concept of conservative therapy. The aim of this work was the comparison of the therapeutic success between balloon kyphoplasty and conservative therapy in the treatment of fresh stable traumatic vertebral body fractures of the thoracic and lumbar spine.

From the results it can be concluded that in the examined patients the outcome of balloon kyphoplasty is superior to that of conservative therapy. This is proven by a higher body function status in the activities of daily life, as well as a lower pain intensity and can be further demonstrated by the improvement of objective radiomorphological parameters.

Migration und Outcome des Aesculap-Schraubrings SC

Zusammenfassung: Das Ziel dieser Studie war es, neben der Migrationsrate des Aesculap-Schraubrings SC auch das allgemeine und gelenkspezifische Outcome von Patienten 10 Jahre nach zementfreiem und zementiertem Hüftgelenkersatz zu evaluieren.

Um Daten bzgl. des allgemeinen Outcomes zu gewinnen, wurden die Ergebnisse des SF-36-Lebensqualitätsscores und des Duke-Gesundheitsprofils ausgewertet. Mithilfe des Scores nach Merle D’Aubigné und Postel, des Harris-Hip-Scores und des Mayo-Hip-Scores wurden Informationen zum gelenkspezifischen Outcome erhoben.

Die Migration des Schraubrings wurde mit dem Computerprogramm der Ein-Bild-Röntgen-Analyse (EBRA) bestimmt. Bei 15 von 26 Patienten wurde mittels EBRA eine Migration von mehr als 1mm nachgewiesen. Es konnte kein statistisch signifikanter Unterschied bzgl. des allgemeinen und des gelenkspezifischen Outcomes in den Gruppen mit Migration (Migrationsgruppe: MG) gegenüber der Gruppe ohne Migration (Nicht-Migrationsgruppe: NMG) festgestellt werden. Der SF-36-Lebensqualitätsscore zeigte 49 Punkte (23/60) in der MG gegenüber 45 Punkten (30/60) in der NMG. Das Duke-Gesundheitsprofil zeigte ein Überwiegen der positiven Werte für beide Gruppen. Im Score nach Merle D‘Aubigné und Postel erzielte die MG 16 (7/18) Punkte, die NMG 15 (10/18) Punkte. Im Harris-Hip-Score erhielt die MG 91 (43/98) Punkte, während die NMG lediglich 85 (50/95) Punkte erreichen konnte. Einen ähnlichen Wert wiesen beide Gruppen im Mayo-Hip-Score auf, 77 (34/80) Punkte in der MG versus 70 (34/79) Punkte in der NMG.

Die Studie konnte zeigen, dass mit einem Schraubring SC versorgte Patienten gute Ergebnisse im Allgemeinen und gelenkspezifischen Outcome 10 Jahre nach der OP vorweisen konnten, wobei das Ausmaß der Migration des Schraubrings hierbei keinen negativen Einfluss zu nehmen scheint.

Summary: The aim of this study was to explore the migration of the Aesculap Screwcup SC and to evaluate the general and the joint specific outcome ten years after cementless and hybrid total hip arthroplasty.

To get data of the general-outcome, the results of the SF-36 and the Duke-Health-Profile were evaluated. With the scores of Merle D’Aubigné and Postel, Harris-Hip-Score and Mayo-Hip-Score information about the joint-specific-outcome was received. The stability of the cup was evaluated by the computer program EBRA.

In 15 out of 26 patients, a migration of more than 1 mm could be verified by EBRA. There was no statistically significant difference of the general and joint specific outcome of the patients with migration (migration group: MG) compared to those without migration (non-migration group: NMG).The SF-36 showed 49 points (23/60) in the MG and 45 points (30/60) in the NMG. The Duke-Score showed a predominance of the positive scales for both groups. In the score of Merle D’Aubigné the MG reached 16 points (7/18), the NMG 15 points (10/18). In the Harris-Hip-Score the results were 91 points (43/98) (MG) versus 85 points (50/95) (NMG). A similar result was revealed in the Mayo-Hip-Score with 77 points (34/80) in MG versus 70 points (34/79) in the NMG.

The study could demonstrate that patients with a Screwcup SC showed good general and joint specific outcomes ten years after surgery, whereas the size of migration of the cup did not seem to have a negative influence.

Ergebnisse nach zweizeitigem Knieendoprothesenwechsel mit artikulierenden und nicht-artikulierenden Knochenzement-Antibiotikum Platzhaltern

Zusammenfassung

Hintergrund: Nach Implantation von Knieendoprothesen ist der zweizeitige Prothesenwechsel beim periprothetischen Spätinfekt die bevorzugte Therapieoption. Für den Zeitraum bis zur Infektsanierung (Interimsphase) werden Knochenzement-Antibiotikum-Platzhalter (Spacer) verwendet. Nichtartikulierende Blockspacer werden von artikulierenden Spacern unterschieden.

Fragestellung: Ziel der Arbeit war die retrospektive Analyse der Behandlungsergebnisse des zweizeitigen Prothesenwechsels unter Verwendung von intraoperativ handgeformten Blockspacern im Vergleich zu artikulierenden Spacern.

Material und Methoden: Es wurden 46 Patienten mit 47 Endoprothesen inkludiert, die sich über einen Zeitraum von 9 Jahren einem zweizeitigen Prothesenwechsel unterzogen. Hierbei wurden 32 artikulierende Spacer und 15 Blockspacer implantiert. Der mittlere Nachbeobachtungszeitraum betrug 55 (12/145) Monate.

Ergebnisse: Die klinische Untersuchung nach Reimplantation ergab keine signifikante Verbesserung des Bewegungsausmaßes durch Verwendung artikulierender Spacer (artikulierende Spacer 0/0/100°; Blockspacer 0/0/90°). Die Infektsanierung war bei Verwendung artikulierender Spacer signifikant (p=0,015) schneller zu erreichen (artikulierende Spacer 10,75 Wochen; Blockspacer 12 Wochen). Unabhängig von der Versorgungstechnik wurde eine hohe Reinfektionsrate beobachtet (artikulierende Spacer 28%; Blockspacer 27%; p=1,0). Die Rate der persistierenden Infektionen in der Interimsphase unterschied sich ebenso nicht signifikant (artikulierende Spacer 13%; Blockspacer 7%; p=0,66). Zum Nachuntersuchungszeitpunkt waren 8 (17%) Patienten mit einer Arthrodese versorgt. Nach Blockspacerversorgung war mit 14% gegenüber 6% nach Versorgung mit einem artikulierenden Spacer eine höhere prozentuale Häufigkeit der sekundären Arthrodesen festzustellen.

Schlussfolgerungen: Die Verwendung artikulierender Spacer verbessert in unserem Patientengut die spätere Gelenkfunktionalität nicht signifikant. Die Behandlung des Protheseninfekts zählt insbesondere aufgrund der hohen Reinfektionsrate zu den optimierbaren Verfahren in der Alloarthroplastik des Kniegelenks

Summary

Background: The two-stage exchange of total knee arthroplasty for late-onset periprosthetic infection is the gold standard in the septic revision of total knee arthroplasty. Antibiotic-loaded cement spacers are commonly used for the two-stage exchange procedure. We distinguish between non-articulating blockspacers and articulating spacers.

Objectives: The aim of the study was to retrospectively evaluate the results of two-stage exchange of total knee arthroplasty by using blockspacers or articulating spacers.

Materials and methods: The study included 46 patients (47 endoprostheses) with a septic two-stage exchange revision from 1999 to 2008. 32 articulating spacers and 15 blockspacers were implanted. The mean follow-up period was 55 (12/145) months.

Results: The clinical examination did not show a significant improvement of the range of motion when articulating spacers or blockspacers were used (articulating spacer 0/0/100°; blockspacer 0/0/90°). The period until the infection was cured was significantly (p=0.015) shorter when articulating spacers were used (articulating spacers 10.75 weeks; blockspacers 12 weeks). The scores which evaluated the quality of life showed that physical functions were more affected than psychic factors in comparison with the general population. Independent of the technique of treatment, a considerable reinfection rate was found (articulating spacers 28%, blockspacers 27%; p=1.0). The rate of persistent infections during the interim phase was not influenced by the technique of treatment either (articulating spacers 13%; blockspacers 7%; p=0.66). At the time of the follow-up examination, 8 (17%) patients were treated with arthrodesis. We found that the implantation of blockspacers imply a higher percentage of secondary knee arthrodeses (articulating spacers 6%; blockspacers 14%).

Conclusion: The use of articulating spacers does not significantly improve the subsequent joint function in our patients. The results of this study demonstrate that the two-stage exchange of total knee arthroplasty must be improved in future, especially with respect to high reinfection rates.
