Zusammenfassung: Die Schultersteife ist ein klinisches Syndrom mit Einschränkung der aktiven und passiven Beweglichkeit des Schultergelenks. Entscheidend ist die Unterscheidung zwischen primärer und sekundärer Schultersteife, da sich Ursachen, Verlauf und Therapie unterscheiden. Die primäre Schultersteife ist eine meist selbstlimitierende entzündlich-fibrotische Erkrankung der Gelenkkapsel mit einem typischen Verlauf aus Schmerz-, Steife- und Erholungsphase. Die sekundäre Schultersteife entsteht dagegen nach Trauma, Operationen, Frakturen, Rotatorenmanschettenrekonstruktionen, Schulterstabilisierungen, Kalktendinitis, etc. Neben kapsulären Kontrakturen bestehen häufig extrakapsuläre Adhäsionen im subakromialen, subdeltoidalen und subkorakoidalen Raum. Während die primäre Schultersteife häufig konservativ ausheilt, sind die Ergebnisse bei sekundären Formen oft begrenzt. Bei persistierender, funktionell relevanter Steife stellt die arthroskopische Arthrolyse ein erfolgreiches operatives Verfahren dar. Dabei werden intraartikulär die kontrakten Anteile der Gelenkkapsel einschließlich des Rotatorenintervalls sowie der anterioren, inferioren und gegebenenfalls posterioren Kapsel gelöst. Zusätzlich erfolgt bei sekundären Formen häufig eine extraartikuläre Arthrolyse mit Lösung subakromialer, subdeltoidaler und subkorakoidaler Verwachsungen, um die physiologische Gleitfähigkeit der Weichteile wiederherzustellen. Wichtig sind auch begleitende operative Verfahren, wie bspw. eine Manschettenrekonstruktion, Implantatentfernung, Kalkentfernung sowie die Versorgung von Bizepssehnenpathologien. Zahlreiche Studien zeigen nach arthroskopischer Arthrolyse sehr gute Verbesserungen von Schmerzen, Beweglichkeit und Schulterfunktion. Viele Patientinnen und Patienten erreichen wieder nahezu normale Alltagsfunktionen und können auch zu Überkopf- und Wurfsportarten zurückkehren. Eine konsequente Physiotherapie nach der arthroskopischen Arthrolyse ist entscheidend.
Summary: Shoulder stiffness is a clinical syndrome characterized by restricted active and passive range of motion of the shoulder joint. The distinction between primary and secondary shoulder stiffness is essential, as the causes, clinical course, and treatment differ. Primary shoulder stiffness is a predominantly self-limiting inflammatory-fibrotic disorder of the joint capsule with a typical course consisting of a painful phase, a stiffness phase, and a recovery phase. In contrast, secondary shoulder stiffness develops following trauma, surgery, fractures, rotator cuff repairs, shoulder stabilization procedures, calcific tendinitis, etc. In addition to capsular contractures, extracapsular adhesions are frequently present in the subacromial, subdeltoid, and subcoracoid spaces. While primary shoulder stiffness often resolves with conservative treatment, the results in secondary forms are frequently limited. In cases of persistent, functionally relevant stiffness, arthroscopic arthrolysis represents a successful surgical treatment option. During this procedure, the contracted parts of the joint capsule, including the rotator interval as well as the anterior, inferior, and, if necessary, posterior capsule, are released intra-articularly. In addition, secondary forms frequently require extra-articular arthrolysis with release of adhesions in the subacromial, subdeltoid, and subcoracoid spaces in order to restore the physiological gliding capacity of the soft tissues. Concomitant surgical procedures are also important, such as rotator cuff reconstruction, implant removal, excision of calcific deposits, and treatment of biceps tendon pathologies. Numerous studies have demonstrated very good improvements in pain, range of motion, and shoulder function following arthroscopic arthrolysis. Many patients regain nearly normal function in daily activities and are able to return to overhead and throwing sports. Consistent physiotherapy following arthroscopic arthrolysis is essential.
Zusammenfassung: Die inverse Schultertotalendoprothese (RSA) hat sich als sehr wirksame Behandlungsoption sowohl bei primären als auch sekundären knöchernen Defekten im Revisionsfall erwiesen. Nach einer gescheiterten inversen Prothese können ausgeprägte Defekte im Glenoid vorliegen. Diese Übersichtsarbeit fasst rekonstruktive Optionen zum Wiederaufbau eines glenoidalen Knochendefekts und knöcherne Aufbautechniken mit Auto- oder Allografts zusammen. Auch der Einsatz metallisch augmentierter Basisplatten sowie moderne patientenindividuelle Implantate aus dem 3D-Druck sind möglich. Unter dem Einsatz einer präzisen, präoperativen 3D-Planung kann eine zuverlässige Rekonstruktion mit guten klinischen Ergebnissen, sowohl einzeitig, als auch zweizeitig gelingen.
Summary: Reverse shoulder arthroplasty (RSA) has proven to be a highly effective treatment option for both primary and secondary osseous defects in the revision setting. Following failure of a reverse prosthesis, substantial glenoid defects may be present. This review summarizes reconstructive options for the management of glenoid bone loss, including bony reconstruction techniques using autografts or allografts. The use of metal-augmented baseplates as well as modern patient-specific 3D-printed implants is also feasible. With the aid of precise preoperative 3D planning, reliable reconstruction with good clinical outcomes can be achieved in both one-stage and two-stage procedures.
Zusammenfassung: Bei der anatomischen Schulterendoprothese gilt es vor allem, an die Gefahr der späteren Glenoidlockerung zu denken. Dies gilt vor allem für die Versorgung jüngerer Patientinnen und Patienten mit Early-Onset-Arthrose. Dazu wurden in der Vergangenheit multiple Verankerungskonzepte vorgestellt, teilweise aber mit deutlich schlechteren Ergebnissen als bei herkömmlichen zementierten Polyethylenen. Moderne konvertierbare Implantate mit einem „Full-metal“ Glenoid und trabekulärer, metallischer Verzapfung sind sehr vielversprechend und scheinen gegenüber zementierten Polyethylen-Pfannen deutliche Vorteile zu bieten. Insbesondere bei starker Retroversion und bikonkavem Glenoid (B2) können sie die Augmentation des Defektes vereinfachen. Zur präzisen Komponentenplatzierung und bei Fällen mit knöcherner Korrektur wird eine präoperative 3D-Planung dringend empfohlen.
Summary: In anatomical shoulder arthroplasty, it is important to consider the risk of subsequent glenoid loosening. This applies particularly to the treatment of younger patients with Early-Onset-Osteoarthritis. Multiple anchoring concepts have been used in the past; however, some with poorer results than those with cemented polyethylenes. Modern, convertible implants with a “full-metal” glenoid and trabecular, metallic pegs seem promising and appear to have advantages over conventional systems with a cemented polyethylene. They can simplify bony augmentation of a glenoid defect, particularly in severe retroversion or biconcave glenoids (B2). Preoperative 3D-planning is strongly recommended to aid the surgeon in component placement and for augmentation purposes.
Zusammenfassung: Die Omarthrose zeigt ein vielschichtiges klinisches Bild. Die Therapiemöglichkeiten sind nicht minder vielfältig. Je nach den erhobenen Befunden zeigen rein arthroskopische gelenkerhaltende Therapieoptionen u./o. konservative Möglichkeiten sehr gute Ergebnisse. Hierbei bedarf die/der Arthrosepatientin/-patient einer individuellen Abwägung der gelenkerhaltenden Operation oder eines Gelenkersatzes. Aktuell anerkannte Verfahren wie die arthroskopische CAM (Comprehensive Arthroscopic Management)- Prozedur an der Schulter sind anspruchsvoll und bedürfen ein differenziertes, individuell ausgerichtetes Vorgehen. Für diese gelenkerhaltenden Verfahren gilt, je früher, desto besser. Ein Herauszögern schmälert die Ergebnisse dieser Operationen. Der frühe korrigierende Eingriff in Kombination mit einer konservativen, individuell ausgerichteten Therapie ist für viele Patientinnen und Patienten das am ehesten erfolgreiche Konzept.
Summary: Omarthrosis has a complex clinical appearance. Therapy options are no less diverse. Depending on the clinical appearance, arthroscopic joint-preserving treatment possibilities and/or conservative options show very good results. The shoulder osteoarthritis patient requires an individual consideration of a joint-preserving operation or a joint replacement. Currently recognized concepts such as the arthroscopic CAM (Comprehensive Arthroscopic Management) procedure on the shoulder are demanding and require a differentiated and individual approach. The principle applying to these joint-preserving procedures is: the sooner the better. A delayed treatment diminishes the surgical results. An early corrective intervention in combination with a customized conservative therapy is the most successful concept for many patients.

Endoprotheseninfektionen der Schulter stellen häufig ein komplexes Problem dar, das in der Regel eine operative Behandlung erfordert. Die typischen Behandlungsalgorithmen hinsichtlich Infektursache, Zeitpunkt und Keimspektrum sind auch bei dem Low-grade-Infekt zu berücksichtigen. Er unterscheidet sich aber von anderen Infekten dadurch, dass er häufig als solcher nicht erkannt wird. Das Therapiespektrum beinhaltet zum einen das prothesenerhaltende Debridement mit Komponentenwechsel sowie den 2-zeitigen, kompletten Prothesenwechsel unter Einsatz einer antibiotikabeladenen Interimsprothese. Von entscheidender Bedeutung ist die Erkennung eines Low-grade-Infekts, da die typischen klinischen Zeichen meist fehlen, und dessen sachgerechte Therapie, um eine Infektpersistenz und damit eine fortschreitende Schädigung des Knochenlagers zu vermeiden.

Summary: An infected shoulder prosthesis is commonly encountered as a complex problem. It is challenging for the patient and the surgeon simultaneously because surgical treatment is mandatory in most of the cases. The treatment algorithms valid for prosthetic infections are applied in a Low-grade infection as well. They orientate on the infective cause, the timepoint of its appearance and the detected microbial spectrum. However, a Low-grade infection is difficult to recognize as such. Therapy options range from a prosthesis-preserving debridement with component replacement to a 2-step revision procedure including total and meticulous foreign body removal and temporary spacer implantation loaded with antibiotics. The detection of a Low-grade infection is the first and crucial step for an effective treatment because the typical infection signs are usually missing. An appropriate and early therapy is necessary in order to prevent the slow growing bacteria from further spread and persistence in the host tissue, because both can lead to progressive and significant bone damage.

Die häufigsten Ursachen für eine Revisionsoperation nach fehlgeschlagener iSTEP sind Instabilität, Infektion und eine Implantatlockerung. Revisionen von iSTEP gelten als technisch schwierige und anspruchsvolle Eingriffe, die ein hohes Maß an Erfahrung, sorgfältiger Vorbereitung und Ausstattung mit speziellen Instrumentarien und Revisionssystemen verlangen. Mehrfache Operationsschritte [6, 13] und die Verwendung von Sonderimplantaten sind nicht selten [13, 23]. Höhere Komplikations- und die Revisionsraten von 20–40%, je nach individueller Ausgangslage und Erfahrung des Operateurs, werden in der Literatur berichtet. Dennoch kann eine Verbesserung zum Ausgangsbefund mit weitgehender Erhaltung der Schulterfunktion bei 90% der Fälle erzielt werden [15].
Summary: The most common cause of revisions performed after reverse shoulder arthroplasty (RSA) are instability, infection and aseptic loosening of the components. Revisions of RSA are demanding and require a meticelous preoperative planning, special intrumentation and revision implants besides a surgeon who is very experienced in the field of revision shoulder arthroplasty. The failure and revision rate is still considerable high with 20–40% reported in literature. However, in the majority of cases a reasonable improvement of function can be achieved in the majority of cases.
