Morohashi et al. [27] berichteten über einen Fall mit Iliopsoas-Impingement, wiesen auf die Notwendigkeit eines 2-Level-Releases hin und sie beobachteten, dass ein Release im Bereich des Trochanter minor nicht ausreichend war und ein zweites Release auch für die ventrale Pfannenkomponente notwendig wurde. Dieses mag unter dem Aspekt der oben dargestellten anatomischen Zusammenhänge verständlich sein.

Fazit und klinische Relevanz

Das IPI muss in die differenzialdiagnostischen Überlegungen bei anhaltenden Beschwerden nach hüftalloarthroplastischem Gelenkersatz miteinbezogen werden. Die typische Anamnese, Schmerzen bei der kraftvollen Hüftflexion zwischen 0–70° sowie ein positiver LA-Test sind wegweisend für die Diagnose. Konservative Therapieverfahren zeigen nur geringe Aussicht auf Erfolg. Ein arthroskopisches Release der Iliopsoas-Sehne bietet eine minimal invasive Therapieoption mit vorhersagbar gutem Erfolg und ist einer erneuten Prothesenrevision mit Umplatzierung der Komponenten nach dem jetzigen Stand der Literatur vorzuziehen. In vielen Fällen sind mehrere Sehnenanteile zu lösen.
