Medikamentöse Therapie entzündlich rheumatischer Erkrankungen

Zusammenfassung:

Die medikamentöse Therapie entzündlich rheumatischer Erkrankungen hat sich in den letzten 20 Jahren rasant entwickelt. Neue Therapiestrategien in Kombination mit einer großen Anzahl innovativer Medikamente haben die Möglichkeiten der Versorgung der Patienten drastisch verbessert. Rheuma ist kein unabänderliches Schicksal mehr; Zuversicht bei Arzt und Patient. Das Behandlungsziel Remission bzw. niedrige Krankheitsaktivität ist keine Utopie, sondern häufig Realität. Neben den bekannten Medikamenten in der Frühbehandlung wie NSAR, Glukokortikoide und konventionelle DMARDs, haben die Biologika und aktuell auch die JAK-Inhibitoren einen großen Anteil an dieser äußerst positiven Entwicklung.

Summary: The drug therapy of inflammatory rheumatic diseases has developed rapidly in the last 20 years. New therapeutic strategies combined with a large number of innovative drugs have dramatically improved the patients care options. Rheumatism is no longer an unalterable fate; confidence in the doctor and patient. The treatment goal remission or low disease activity is no utopia, but often reality. In addition to the well-known drugs in the early treatment, such as NSAIDs, glucocorticoids and conventional DMARDs, the biologicals and currently also the JAK-inhibitors have a large share in this positive development.

Schulter-Revisionsendoprothetik – Fakten und Zahlen

Zusammenfassung:

Eine Revision nach fehlgeschlagener primärer Schulterendoprothese ist vergesellschaftet mit erschwerter operativer Durchführung, erhöhter Komplikationsrate und schlechteren Ergebnissen. Die Ergebnisse variieren dabei mit der Ursache der Revision. Trotz deutlicher klinischer Verbesserungen zeigen Schulter-TEP-Revisionen häufig schlechte subjektive Zufriedenheitswerte im Vergleich mit typgleichen Primärversorgungen. Zusätzliche Voroperationen an der Schulter scheinen einen negativen Einfluss auf das Revisionsergebnis zu haben. Generell günstigere Voraussetzungen finden sich hingegen bei Infektfreiheit, intakter Rotatorenmanschette (RM), stabilem Gelenk und gutem Knochenlager. Eine Protheseninfektion als Revisionsgrund hat in der Regel die schlechtesten Ergebnisse. Eine inverse Schulter-TEP bietet im Revisionsfall konzeptionell Vorteile bei RM-Defekten und glenohumeraler Instabilität zulasten typspezifischer Komplikationen. Modulare, individuelle und konvertierbare Primärprothesen sollen zukünftig im Revisionsfall umfangreichere anwenderfreundliche Optionen zur Verbesserung der Funktion und Verlängerung der Standzeiten bei verringerter Komplikationsquote bieten.

Summary: Revision after failed primary shoulder arthroplasty correlates with aggravated operative handling, increased risk of complications and worsened results. Results vary by reason. Despite of clinical improvement results of revision shoulder arthroplasty often are inferior in PROM than comparable primary arthroplasties. Additional shoulder operations seem to diminish revision results. In general, absence of infection, intact rotator cuff, joint stability and sufficient bone stock are favourable in revision cases. However, periprosthetic infection is associated with the worst outcome expectancy after revision shoulder arthroplasty. In revision cases, reverse shoulder arthroplasty is advantageous in cuff deficiency and glenohumeral instability by its genuine concept despite of typical complications. Modular, individual and convertible primary shoulder prostheses shall offer additional considerable smart options for functional improvement, long-term survival and reduced failure rate in revision cases in the future.

Revisionsendoprothetik bei Patienten mit rheumatoider Arthritis

Zusammenfassung:

Mit steigenden Operationszahlen für die Primärendoprothetik des Schultergelenks bei Patienten mit einer rheumatoiden Arthritis steigt zwangsläufig auch die Anzahl der Revisionseingriffe. Patienten mit RA haben medikamentös und krankheitsbedingt eine schlechtere Knochen- und Sehnen-/Muskelqualität, was eigentlich zu einer schlechteren postoperativen Funktion und einer höheren Komplikationsrate mit entsprechend hohen Revisionszahlen einhergehen sollte.

Die Literatur zeigt zwar eine teils sehr hohe Rate an radiologischen Auffälligkeiten (Lysesäumen), eine signifikant höhere Revisionsrate als im gemischten Normalkollektiv ist jedoch nicht beschrieben. Dies gilt insbesondere für die postoperative Infektionsrate. Für die perioperative Begleitmedikation existieren mittlerweile Empfehlungen, die jedoch nur einen schwachen Evidenzgrad aufweisen. Es fehlen aussagekräftige Studien für dieses spezielle Patientenkollektiv.

Summary: The numbers of rheumatoid patients requiring artificial joint replacement are rising and therefore rising numbers of revision surgeries can be expected. Authors have previously cautioned against using artificial joint replacement in RA patients due to concerns about increased peri-operative complications, poor functional outcomes and high revision rates due to reduced bone and soft tissue quality. Literature shows a significantly high percentage of radiological abnormalities following implantation of both anatomical and reverse systems. However, both anatomical and reverse designs in RA showed similar short- to mid-term results without higher complication rates as compared to other pathologies. Guidelines are available for the perioperative management of antirheumatic medication in patients with rheumatic diseases undergoing joint arthroplasty. However, they are based on low quality evidence. Further studies are required to determine the long-term survival of the implant.

Revision der inversen Schulterprothese (RSA)

Zusammenfassung:

Die häufigsten Ursachen für eine Revisionsoperation nach fehlgeschlagener iSTEP sind Instabilität, Infektion und eine Implantatlockerung. Revisionen von iSTEP gelten als technisch schwierige und anspruchsvolle Eingriffe, die ein hohes Maß an Erfahrung, sorgfältiger Vorbereitung und Ausstattung mit speziellen Instrumentarien und Revisionssystemen verlangen. Mehrfache Operationsschritte [6, 13] und die Verwendung von Sonderimplantaten sind nicht selten [13, 23]. Höhere Komplikations- und die Revisionsraten von 20–40%, je nach individueller Ausgangslage und Erfahrung des Operateurs, werden in der Literatur berichtet. Dennoch kann eine Verbesserung zum Ausgangsbefund mit weitgehender Erhaltung der Schulterfunktion bei 90% der Fälle erzielt werden [15].

Summary: The most common cause of revisions performed after reverse shoulder arthroplasty (RSA) are instability, infection and aseptic loosening of the components. Revisions of RSA are demanding and require a meticelous preoperative planning, special intrumentation and revision implants besides a surgeon who is very experienced in the field of revision shoulder arthroplasty. The failure and revision rate is still considerable high with 20–40% reported in literature. However, in the majority of cases a reasonable improvement of function can be achieved in the majority of cases.

Low-grade-Infektionen bei Schulterprothesen

Zusammenfassung:

Endoprotheseninfektionen der Schulter stellen häufig ein komplexes Problem dar, das in der Regel eine operative Behandlung erfordert. Die typischen Behandlungsalgorithmen hinsichtlich Infektursache, Zeitpunkt und Keimspektrum sind auch bei dem Low-grade-Infekt zu berücksichtigen. Er unterscheidet sich aber von anderen Infekten dadurch, dass er häufig als solcher nicht erkannt wird. Das Therapiespektrum beinhaltet zum einen das prothesenerhaltende Debridement mit Komponentenwechsel sowie den 2-zeitigen, kompletten Prothesenwechsel unter Einsatz einer antibiotikabeladenen Interimsprothese. Von entscheidender Bedeutung ist die Erkennung eines Low-grade-Infekts, da die typischen klinischen Zeichen meist fehlen, und dessen sachgerechte Therapie, um eine Infektpersistenz und damit eine fortschreitende Schädigung des Knochenlagers zu vermeiden.

Summary: An infected shoulder prosthesis is commonly encountered as a complex problem. It is challenging for the patient and the surgeon simultaneously because surgical treatment is mandatory in most of the cases. The treatment algorithms valid for prosthetic infections are applied in a Low-grade infection as well. They orientate on the infective cause, the timepoint of its appearance and the detected microbial spectrum. However, a Low-grade infection is difficult to recognize as such. Therapy options range from a prosthesis-preserving debridement with component replacement to a 2-step revision procedure including total and meticulous foreign body removal and temporary spacer implantation loaded with antibiotics. The detection of a Low-grade infection is the first and crucial step for an effective treatment because the typical infection signs are usually missing. An appropriate and early therapy is necessary in order to prevent the slow growing bacteria from further spread and persistence in the host tissue, because both can lead to progressive and significant bone damage.

Revision der instabilen Schulterendoprothese und Komplikationen

Zusammenfassung: Das Auftreten einer Instabilität im Zusammenhang mit einer Schulterendoprothese ist ein häufiges klinisches Problem und stellt einen der häufigsten Gründe für eine Revisionsoperation dar. Die Vermeidung auslösender Faktoren bei der Primärimplantation und der nachfolgenden Rehabilitation sind gut beeinflussbare Faktoren, die unbedingt beachtet werden müssen. Totalendoprothesen bei erhaltener Rotatorenmanschette (TSA) und bei insuffizienter Rotatorenmanschette (RSA) zeigen unterschiedliche Muster der Instabilität und der Therapiestrategie. Häufigste operative Therapie der instabilen TSA ist die Konversion auf eine inverse Schulterendoprothese (RSA). Häufige Gründe für instabile RSA sind Implantat-assoziiert bzw. Implantations-assoziiert und entsprechend operativ zu adressieren. Ein glenoidaler Knochensubstanzverlust geht mit der Gefahr einer Instabilität der RSA einher und sollte durch knöchernen Aufbau und/oder lateralisierende Komponenten adressiert werden. Eine Schädigung des Deltamuskels ist desaströs, schwer zu behandeln und muss unbedingt vermieden werden. Revisionsoperationen von instabilen Schulterendoprothesen stellen besondere chirurgische und apparative Anforderungen und sollten zum Wohl der Patienten spezialisierten Zentren vorbehalten bleiben.

Summary: Instability after shoulder arthroplasty procedures is a common problem and one main reason for revision surgery. Avoiding triggering factors during primary implantation and the following rehabilitation can be well addressed and should be a matter of interest. Total shoulder arthroplasties with a remaining rotator cuff (TSA) and those with an insufficient cuff (RSA) present with different patterns of instability and therefore need a specific treatment strategy. The most common surgical procedure for unstable TSA is the conversion to a RSA. Common reasons for unstable RSA are implant- and implantation-associated and should be adressed as such. Glenoid bone loss increases the risk for instability after RSA and should be adressed with bony augmentation and/or lateralizing implant components. Any damage of the deltoid muscle will be disastrous and difficult to treat and therefore should be avoided at all costs. Revision surgery for unstable shoulder arthroplasties are challenging both for the surgeon and the infrastructure. Therefore, these procedures should be reserved for specialized surgeons and treatment facilities.

Craniomandibuläre Dysfunktion – eine oft nicht beachtete Komorbidität des nicht-spezifischen Rückenschmerzes

Methode: Kiefer- und Kopfgelenke bilden eine kybernetische Einheit. Ziel dieser Studie ist es, mögliche Zusammenhänge zwischen der Chronifizierung des nicht-spezifischen Rückenschmerzes und der CMD/CCD zu ermitteln.

Ergebnisse: Von den 652 CMD-CCD-Rückenschmerz-Patienten waren 67,7% der Patienten weiblich, 32,3% männlich. 82,8% erfüllten zusätzlich die diagnostischen Kriterien der AWMF 2012 des Fibromyalgie-Syndroms. Durch die synchrone Therapie der CMD-CCD-FWS konnte bei 85% der bislang therapieresistenten CMD-CCD-Rückenschmerz-Patienten doch noch eine gute Besserung der Beschwerden im Körper und bei 90% eine Besserung der Beschwerden im Kiefer erzielt werden.

Diskussion: Die CMD kann als Folge chronischer Schmerzen auftreten. Umgekehrt kann eine CMD auch muskuloskelettale Störungen hervorrufen oder unterhalten. Bei der Diagnose und der Behandlung des nicht-spezifischen Rückenschmerzes muss auch eine CMD untersucht und mitbehandelt werden.

Abstract

Method: Jaw and head joints form a cybernetic unit. The aim of this study is to determine possible relationships between the chronification of non-specific back pain and the CMD / CCD.

Results: Of the 652 CMD-CCD-back pain patients, 67.7% of the patients were female, 32.3% were male. In addition, 82.8% met the diagnostic criteria of AWMF 2012 for fibromyalgia syndrome. Thanks to the synchronous therapy of the CMD-CCD-FWS, 85% of the previously therapy-resistant CMD-CCD-back pain patients were able to achieve a good improvement of the complaints in the body and 90% an improvement of the complaints in the jaw.

Discussion: CMD can occur as a result of chronic pain. Conversely, CMD may also cause or sustain musculoskeletal disorders. In the diagnosis and treatment of non-specific back pain, a CMD must also be examined and treated.

Klassifikation von Knorpelschäden am Hüftgelenk

Zusammenfassung:

Die präoperative Diagnostik azetabulärer Knorpel- und Labrumläsionen umfasst konventionell radiologische und MR-morphologische Bildgebung. Die morphologische chondrolabrale Defektklassifikation erfolgt anhand der Kriterien von Haddad, die Defektlokalisation anhand der Methode von Griffin. Verschiedene arthroskopische Verfahren stehen zur Verfügung. Die Abrasionsarthroplastik und die Mikrofrakturierung stellen gegenwärtig die Standardverfahren zur Behandlung von vollschichtigen, klein- bis mittelflächigen Knorpelläsionen dar. Beide Verfahren sind kostengünstig durchführbar, können jedoch technisch anspruchsvoll in ihrer Durchführung sein. Die höherwertigen Knorpeltherapien wie AMIC und ACT sind bei Defekten von 2cm² indiziert. Sie stellen anspruchsvolle Operationstechniken dar, die dem erfahrenen Operateur vorbehalten bleiben sollten. Nach der gegenwärtigen Datenlage scheint sich eine Überlegenheit der AMIC und der ACT gegenüber den klassischen knochenmarkstimulierenden Techniken zu zeigen. Zu berücksichtigen ist, dass diese kostenintensiver sind und die ACT ein zweizeitiges Vorgehen erforderlich macht. Essenziell ist bei allen Knorpeltherapien am Hüftgelenk die Mitbehandlung der zugrunde liegenden Pathologie, ohne die eine Knorpelregeneration nicht dauerhaft erfolgreich sein kann.

Summary: The preoperative diagnostic algorithm in case of articular cartilage defects of the acetabulum should include radiographic and MR-imaging. Lesions of the chondrolabral complex are classified using the criteria of Haddad, the defect localisation should be performed using the Griffin method.Several arthroscopic techniques are among the available treatment options. Arthroscopic chondroplasty and microfracture can be seen as gold standard for small- to medium-sized, full-thickness cartilage defects. Both procedures are economical but can be technically demanding. AMIC and ACT should be performed in case of a defect surface 2cm². Both are challenging procedures and should only be performed by experienced arthroscopic surgeons. The current literature suggests superiority of AMIC and ACT to conventional marrow-stimulating techniques. One should consider the higher costs for ACT and AMIC as well as the inevitable two-stage procedure for ACT. Arthroscopic cartilage regeneration can only be successful when surgical treatment includes the correction of the damage cause and addresses concomitant pathologies.

Hüftarthroskopie bei vorliegender Dysplasiehüfte

Zusammenfassung: Symptomatische Fehlbildungen des Hüftgelenks im Adoleszenten- und Erwachsenenalter sind häufig eine Herausforderung in Diagnostik und Therapie. Reorientierende Beckenosteotomien und Umstellungsosteotomien am proximalen Femur sind hier seit Jahrzehnten etablierte OP-Techniken in der gelenkerhaltenden Hüftchirurgie mit guter Datenlage bei publizierten Langzeitverläufen über 30 Jahre [14, 18, 28]. Umstellungsoperationen am Femur und der Hüftpfanne sind komplex, zeitaufwendig in der Nachbehandlung, komplikationsbehaftet und unterliegen einer flachen Lernkurve. Die scheinbar minimalinvasive Arthroskopie wird in den letzten Jahren als alternatives Behandlungsverfahren zu Osteotomien, insbesondere bei „Grenzdysplasien“ und grenzwertigen Retrotorsionsfehlstellungen des Femurs eingesetzt [1, 2, 18]. Bei diesen Indikationen wird die Arthroskopie jedoch kontrovers diskutiert, da die zugrunde liegende Pathologie nicht beseitigt wird und die Studienlage uneinheitlich ist [11, 17, 26, 30]. Kombinierte Fehlbildungen von Pfannendysplasie und femoroazetabulären Impingement (FAI) sind häufig, weshalb die Verfahren häufig nicht in Konkurrenz stehen, sondern additiv angewendet werden können. Eine genaue Kenntnis und Analyse der Pathomorphologie und das Beachten von Co-Faktoren hilft hier, Fehlschläge zu vermeiden.

Summary: Symptomatic development dysplasia of the hip in adolescents and adults are often a challenge in diagnosis and therapy. Reorienting pelvic osteotomies and osteotomies on the proximal femur have been established here for decades. Surgical techniques in joint-preserving hip surgery with good data is published long-term courses over 30 years [14, 18, 28]. Osteotomies of the femur and acetabulum are complex, complicating, time-consuming in rehabilitation. Learning this procedure has a flat learning curve. The supposed „minimally invasive“ arthroscopy has been used in recent years as an alternative method of treatment for osteotomies, especially in „border line dysplasia“ and marginal retro torsion deformity of the femur [1, 2, 18]. However, for this indication, arthroscopy is controversial because the underlying pathology is not resolved and results are inconsistent [11, 17, 26, 30]. Combined development dysplasia of the hip and femoroacetabular impingement (FAI) are common, therefore the procedures are often not in competition but can be used additively. An accurate knowledge and analysis of the pathomorphology and consideration of co-factors helps to avoid failures.

Diagnostik und erfolgreiche Therapie des extraartikulären Hüftimpingements

Zusammenfassung:

In den vergangenen 20 Jahren haben die Diagnostik und Behandlung des intraartikulären femuroazetabulären Hüftimpingements Typ Cam oder Pincer einen festen Stellenwert in der gelenkerhaltenden Hüftchirurgie bekommen. Weit weniger bekannt sind dagegen die unabhängig vom oder gemeinsam mit dem intraartikulären Impingement vorliegenden extraartikulären Impingementsyndrome am Hüftgelenk. Sowohl in der klinischen als auch in der bildgebenden Diagnostik werden diese Pathologien daher als Auslöser von wiederkehrenden Hüftbeschwerden insbesondere bei jungen und aktiven Patienten häufig übersehen. In dieser Arbeit sollen die 5 bekannten extraartikulären Impingementpathologien am Hüftgelenk hinsichtlich ihrer Ätiologie, der klinischen Diagnostik und der möglichen sinnvollen Therapiemaßnahmen beschrieben werden. Es sind dies das ischiofemorale Impingement als Kompressionssyndrom des Musculus quadratus femoris zwischen dem Trochanter minor und dem Sitzbein (1), das subspinale Impingement als mechanischen Konflikt zwischen einer vergrößerten oder fehlorientierten Spina iliaca anterior inferior und dem distalen anterioren Schenkelhals (2), das liopsoasimpingement als Konflikt zwischen der Psoassehne und dem vorderen Labrum azetabulare (3), das tiefe Glutealsyndrom als mechanisches Kompressionssyndrom des Nervus Ischiadicus (4) und das pectineofoveale Impingement als Konflikt zwischen der medialen Plica Synovialis und der Zona orbicularis (5).

Summary: In the last twenty years, the diagnosis and treatment of intraarticular femoroacetabular impingement type Cam or Pincer gained a significant position in joint preserving hip surgery. Much less known in the daily treatment of hip pathologies are the different types of extraarticular hip impingement syndromes. Therefore they are often overlooked in the clinical and radiological diagnosis especially in young and active patients. This article describes the etiology, clinical diagnosis and treatment options of five different types of extraarticular hip impingement: the ischiofemoral impingement as a compression of the quadratus femoris muscle between the lesser trochanter and the ischial tuberosity (1), the subspine impingement as a mechanical conflict between the enlarged or malorientated inferior iliac spine and the distal anterior femoral neck (2), the iliopsoas impingement as a conflict between the iliopsoas and the labrum resulting in distict labral pathology (3), the deep gluteal syndrome as a pain in the buttock due to the entrapment of the sciatic nerve in the deep gluteal space (4) and the pectineofoveal impingement as a conflict between the medial plica synovialis and the zona orbicularis (5).
